 한국의 코로나19 누적 확진자 수가 1400만 명에 육박하면서 대한민국 국민의 약 4분의 1이 코로나19 확진을 경험하게 되었다. 이에 따라 확진 및 격리 기간 동안 사람들이 겪은 다양한 증상과 후유증이 보고되고 있다. 연예인 박명수는 지난 310라디오쇼에서 목이 너무 아프고 코 안에 누가 주먹을 넣어둔 것 같다. 완치된 지 3주가 지났는데 코 맹맹한 소리가 아직도 난다.’라며 코로나19의 고통과 후유증에 대해 언급했다. 또한, 한 설문조사에 따르면, 면밀한 연구가 필요하겠지만 코로나19 확진자의 19.1%가 후유증으로 의료기관을 방문한 경험이 있으며, 최대 79%가 완치 후에도 피로감, 건망증 등을 겪고 있다고 설명하기도 했다. 이 기사에서는 코로나19의 다양한 후유증과 그 후유증이 발생하는 기전에 대해 알아보고자 한다.

 

코로나19 감염 기전

 코로나19 바이러스의 정식 명칭은 SARS-CoV-2이며, 2002년 사스(Severe Acute Respiratory Syndrome, 중증급성호흡기증후군)를 일으킨 바이러스와 동일 계통으로 알려져 있다. 질병명에서 알 수 있듯이 호흡기에 문제를 발생하는 바이러스로 감염 경로도 주로 호흡기이다. 모든 바이러스가 그렇듯이 SARS-CoV-2의 표면에는 spike 단백질이 존재한다. spike 단백질과 결합하는 것이 폐포세포, 후각 상피세포 등의 호흡기에 많이 존재하는 ACE-2(Angiotensin Converting Enzyme-2) 수용체이다. Spike 단백질과 ACE-2의 상호작용을 통해 바이러스가 세포 내로 침입하여 증식을 시작하고 감염이 되는 것이다.

 

후각과 미각에 미치는 영향

 코로나19 발생 초기에는 후각 및 미각 상실이 특징적인 증상으로 나타났다. SARS-CoV-2가 후각과 미각 상피세포의 ACE-2 수용체를 통해 침입하여 증식하면, 우리 몸에서는 이 바이러스들을 잡아먹는 대식세포(Macrophage)가 활성화된다. 대식세포는 바이러스를 잡아먹으면서 다양한 염증 반응을 일으키는 물질을 방출하는데, 이때 IL-6(Interleukin-6)라는 인자가 후각과 미각에 영향을 미친다는 연구 결과가 있다.

 

 IL-6는 직접적으로 염증인자 cascade를 일으켜서 후각세포의 사멸을 유도해 기능을 차단하거나, 간접적으로 신경세포와 그 주변의 교세포의 활성을 조절하면서 후각을 차단하는 작용을 할 것이라고 밝혀졌다. 또한, 통계적 연구자료 분석에서 정상인과의 IL-6 level 차이와 후각 상실 정도가 일정 수준의 상관관계가 있음을 파악했다. 장기적 후각 상실을 분석한 연구에서는 이러한 면역반응이 후각 상피 세포의 염색체 내 DNA에 영향을 주어 후각 수용체 형성과 관련된 유전자 발현을 하향 조절하여 일어나는 것이라고 설명한다.

 

 미각의 상실은 연구마다 다양한 가설로 설명하고 있다. SARS-CoV-2가 혀 상피세포에 존재하는 sialic acid 수용체와 결합해서, 맛을 자극하는 분자가 혀 맛봉오리의 미세한 구멍(taste pore)을 통해 세포 내부로 들어가지 못해 맛을 느낄 수 없을 것이라는 주장이 있다. 다른 가설에서는 맛이라는 감각 자체가 후각에 의존하는 부분이 상당히 크기 때문에, 후각 상실이 미각 상실까지 이어진다고 설명한다.

 

 일각에서는 바이러스가 뇌에 직접 침입하여 후각, 미각을 담당하는 영역에 영향을 주어 감각 상실을 일으킨다는 주장을 하고 있다. 하지만, 뇌에는 SARS-CoV-2가 침입하도록 하는 ACE-2 수용체가 없고, 완치된 후에 늦어도 몇 주 내에 후각과 미각이 대부분 재생된다는 점에서 이러한 주장을 반박하고 있다.

 

뇌 영향(Brain fog) 및 롱 코비드(Long Covid)

 최근 논문에서 대두되고 있는 것이 Brain fog이다. Brain fog는 의학적, 과학적 용어는 아니지만 무엇인가를 생각할 때, 생각이 둔하고, 흐릿하고, 명확하지 않을 때 느끼는 감정을 설명하는 용어이다. 이러한 감정은 꼭 아플 때만 느끼는 것이 아니라, 평소에도 졸리거나 약에 의한 부작용 등으로 멍할 때 느끼기도 한다. 하지만 평소에 느끼는 이 현상은 시간이 지나면 정상으로 되돌아온다.

 

 반면, 코로나19에 의한 brain fog는 뇌손상에 의한 것이며, 심할 경우 뇌졸중까지 이어질 수 있다는 연구결과가 발표되었다. 중국의 한 연구 그룹에 따르면, 코로나19 감염에서 완전히 회복된 것으로 생각되는 29명의 인지 기능에 대한 여러 측면을 조사한 결과, 지속적인 주의력 부분에서 손상을 발견했고 이것은 뇌에서의 산소부족과 염증반응에 기인한 것으로 추측되었다.

 

 영국 통계청(ONS)은 최근 코로나19 감염 이후 이러한 Brain fog를 겪는 환자는 150만 명으로 추산되며 이 환자의 절반은 최소 1년간 기타 후유증이 지속됐다고 보고했다. 이렇듯 적어도 2개월, 통상 3개월 이상 피로감, 호흡곤란, 건망증, 수면장애, 기분장애 등을 겪는 것을 롱 코비드(Long Covid)라고 한다. 우리나라의 중앙방역대책본부는 감염자 1,000명을 대상으로 롱 코비드 추적 조사를 진행할 것이며, 코로나19로 인한 건강 피해 분석과 예방 및 치료법 개발에 빅데이터를 제공한다고 밝혔다.

 한국의 코로나19 확진자 수가 지난 222일 이후 연일 수십 만명에 이르고 있다. 316일에는 약 62만 명으로 최고치를 달성했고 그 이후 점차 줄어드는 추세이나 아직 10만 명대를 유지하고 있다. 미국의 WSJ(The Wall Street Journal)한국이 코로나19 팬데믹에서 엔데믹(풍토병) 수준으로 전환하는 세계 최초의 국가가 될 가능성이 높다.’330일 보도했다. 우리나라는 최근 오미크론 변이로 감염자 수가 급증했지만, 높은 백신 접종율을 바탕으로 누적치명률 약 0.12%를 기록하면서 매우 낮은 수치를 유지하고 있다. 이러한 평가들을 종합하여 실제로 한국 정부는 4월 25일자로 코로나19를 전염병 등급체계 1등급에서 2등급으로 하향 조정하였다.

 

 코로나19가 엔데믹이 된다면, 감염에 대한 방역 조치보다는 감염 및 완치자의 치료 및 후유증 관리에 집중할 필요가 있을 것이다. 코로나19로 인해 일어날 수 있는 후유증을 자세히 파악하고 그 증상을 완화할 수 있는 치료법과 사용가능한 약물 등을 연구하여 치료 알고리즘을 확립한다면, 가까운 미래에는 인플루엔자 정도의 관리로 충분하지 않을까 하는 기대를 해본다.

 

COVID-19 sequelae and mechanism studies

It's been three weeks since I was cured, and I still have a stuffy nose.” he said, referring to the pain and aftereffects of COVID-19. In addition, according to a survey, 19.1% of COVID-19 confirmed patients have visited medical institutions due to aftereffects, and up to 79% are suffering from fatigue and forgetfulness even after complete recovery. In this article, I would like to learn about the various aftereffects of COVID-19 and the mechanism by which the aftereffects occur.

 

<Mechanism of COVID-19 infection>

 The official name of the COVID-19 virus is SARS-CoV-2, and it is known to be the same lineage as the virus that caused the SARS (Severe Acute Respiratory Syndrome) in 2002. As the name of the virus suggests, it is a virus that causes problems in the respiratory tract, and the route of infection is mainly respiratory. As with all viruses, there is a spike protein on the surface of SARS-CoV-2. Binding to this spike protein is the ACE-2 (Angiotensin Converting Enzyme-2) receptor, which is common in respiratory tracts such as alveolar cells and olfactory epithelial cells. Through the interaction of Spike protein and ACE-2, viruses invade into cells, start proliferation, and people become infected.

 In the early stages of COVID-19, loss of smell and taste was a characteristic symptom. When SARS-CoV-2 invades and proliferates through ACE-2 receptors in olfactory and taste epithelial cells, macrophages that prey on these viruses are activated in our body. Macrophages release substances that cause various inflammatory reactions while eating viruses, and studies show that a factor called IL-6 (Interleukin-6) affects smell/taste.

 

 IL-6 is said that it will directly cause inflammatory factor cascade to induce the death of olfactory cells, blocking their function, or indirectly controlling the activity of nerve cells and glial cells around them and blocking their sense of smell. In addition, in the analysis of statistical research data, it was found that there is a certain level of correlation between the IL-6 level difference and the degree of loss of smell with normal people. A study analyzing long-term olfactory loss explains that this immune response affects the DNA in the chromosomes of olfactory epithelial cells and occurs by lowering gene expression related to olfactory receptor formation.

 

 Loss of taste is explained by various hypotheses for each study. It is argued that SARS-CoV-2 binds to the sialic acid receptor present in tongue epithelial cells, so that molecules that stimulate taste cannot enter the cells through the taste buds of the tongue. Another hypothesis explains that the sense of taste itself depends on the sense of smell, so loss of smell leads to loss of taste.

 

 Some argue that viruses invade the brain directly and affect the area responsible for smell and taste, causing sensory loss. However, this argument is refuted in that there is no ACE-2 receptor in the brain that causes SARS-CoV-2 to invade, and most of the sense of smell and taste are regenerated within a few weeks at the latest after complete recovery.

 

<Brain fog and long Covid>

 Brain fog is emerging in recent papers. Brain fog is not a medical or scientific term, but it is a term that describes emotions you feel when you think about something, when you are slow-witted, blurred, or unclear. This feeling is not necessarily felt only when you are sick, but also when you are sleepy or spaced out due to side effects from medicine. However, this phenomenon that I usually feel returns to normal over time.

 

 On the other hand, studies have shown that brain fog caused by COVID-19 is caused by brain damage and can lead to stroke in severe cases. According to a Chinese research group, several aspects of cognitive function in 29 people who are believed to have fully recovered from COVID-19 infection found damage in the continuous attention area, which is presumed to be due to lack of oxygen and inflammatory reactions in the brain.

 

 The Office for National Statistics (ONS) of UK recently reported that 1.5 million patients suffer from such brain fog after the COVID-19 infection, and half of these patients have continued to suffer from other aftereffects for at least a year. As such, experiencing fatigue, shortness of breath, forgetfulness, sleep disorders, and mood disorders for at least two months, usually more than three months, is called "Long Covid." Korea's Central Disease Control Headquarters also announced that it will conduct a long COVID follow-up survey of 1,000 infected people and provide big data for health damage analysis, prevention, and treatment development caused by COVID-19.

 The number of COVID-19 confirmed patients in Korea has reached hundreds of thousands every day since February 22. On March 16, it reached a peak of about 620,000 people, and has been gradually decreasing since then, but it has remained in the 100,000 mark. The Wall Street Journal (WSJ) of the United States reported on March 30, "Korea is likely to become the world's first country to switch from COVID-19 pandemic to endemic." In Korea, the number of infected people has soared recently due to ‘omikron’ variants, but based on a high vaccination rate, the cumulative mortality rate is about 0.12%, maintaining a very low level. Taking these evaluations together, the Korean government actually lowered COVID-19 from first grade to second grade of the infectious disease rating system as of April 25.

 

 If COVID-19 becomes an endemic, what should be focused on in the future will be the treatment of infections and cured patients and the management of aftereffects rather than quarantine measures against infection. If we understand the aftereffects of COVID-19 in detail and study treatments and available drugs to alleviate the symptoms to establish a treatment algorithm, we expect that soon, the level of influenza management will be sufficient on COVID-19.
