2019년도 노벨생리의학상

 

2019년도 노벨생리의학상 수상의 영예는 윌리엄 케일린 미국 하버드대 교수, 피터 랫클리프 영국 옥스퍼드대 교수, 그리고 그레그 세멘자 존스 홉킨스대 교수에게 공동으로 돌아갔다. 세 연구자의 업적은 ‘세포가 산소의 농도에 어떻게 적응하는지’를 밝혀낸 것이다. 동물의 삶에 꼭 필요한 연료인 산소는 생명체가 음식을 연료로 전환하는 과정에 직접적으로 관여한다. 기체중의 산소를 호흡과정으로 몸 안에 받아들이면, 혈액 속의 적혈구가 산소를 운반하여 필요한 조직에 공급한다. 만일 산소가 부족한 환경에 놓이거나, 과격한 운동을 하거나, 질병 등의 상황으로 산소 공급이 원활하지 않다면 몸은 이에 대한 보상 반응을 시작하게 된다. 그렇다면 산소가 부족하다는 신호는 어디서, 누가 보내는 것일까?

 

 

 

 

 

EPO(적혈구 생성 촉진 인자, Erythropoietin)는 신장에서 만들어지며 적혈구의 양을 조절한다. 산소가 부족한 상황(hypoxia)에서 조직에 산소를 원활하게 공급하기 위해서는 적혈구의 수가 증가해야 한다. 그레그 세멘자 교수는 이러한 조절 과정에 기여하는 구체적인 유전자를 밝혀냈다. 또한 그 유전자 조각이 EPO 유전자 바로 옆에 존재하며 hypoxia 상황에 관여함을 알게 되었다. 당시 같은 주제로 연구하고 있던 피터 랫클리프 교수도 그레그 세멘자 교수와 더불어 이러한 조절 기작이 신장 세포뿐 아니라 모든 세포에서 일어나는 세포의 기본적인 기능이라는 것을 발견했다.

그레그 세멘자 교수는 hypoxia에서의 반응을 매개하는 세포 구성물을 구체적으로 밝히고 싶어했다. 그는 간 세포를 배양하여 hypoxia 상황에서 앞선 연구에서 밝혀낸 유전자 조각에 결합하는 단백질 복합체를 발견한 뒤, 이를 HIF(Hypoxia-Inducible Factor)라고 명명하였다. 이후 추가적인 연구를 진행하여 HIF는 HIF-1α와 ARNT라는 두 단백질로 이루어져있다는 사실과 HIF를 부호화하는 유전자를 규명했다.

 

 HIF-1α의 양은 산소의 농도에 반비례한다. 산소가 부족한 상황이 닥치면 HIF-1α의 양이 많아지며, 핵 속에서 ARNT 단백질과 함께 HRE(Hypoxia-Regulated Element) 유전자에 결합해 EPO의 발현량을 증가시킨다. 만약 산소가 부족하지 않은 상황이라면 유비퀴틴이라는 작은 단백질이 HIF-1α와 결합해 프로테아좀(단백질분해효소복합체, proteasome)이 빠르게 HIF-1α를 분해시킬 수 있도록 도와준다.

(1)Hypoxia 상황에서HIF-1α는 분해되지 않으며, ARNT와 함께 Hypoxia-regulated genes의 특정 DNA 서열인 HRE(hypoxia-responsive elements) 에 결합한다. (2) 정상 산소 농도에서는 프로테아좀이 빠르게 HIF-1α를 분해한다. (3) 이때 산소가 HIF-1α에 OH의 형태로 결합하며 (4)VHL이 HIF-1α를 인식하고 복합체를 형성하여 분해 과정을 마무리한다.

 

 

HIF-1α가 분해되는 구체적인 기작은 당시 유전질환과 암을 연구하고 있던 윌리엄 케일린 하버드대 교수가 밝혀냈다. 그는 유전질환인 폰 히펠-린다우 혈액암(VHL disease)을 연구하며 VHL 유전자에 문제가 있는 경우 특정 암에 걸릴 확률이 높다는 것을 발견했다. VHL 유전자는 암의 발병을 저해하는 단백질을 부호화하고 있으며, 암세포 중 VHL 유전자가 부족한 경우 hypoxia-regulated genes를 많이 발현한다는 사실도 발견하였다. 다른 연구 결과는 VHL이 단백질에 유비퀴틴이 결합하여 프로테아좀에서 분해되도록 하는 역할을 한다는 것을 밝혔다. 종합적으로 랫클리프 교수와 연구 그룹은 VHL이 HIF-1α와 상호작용하며 분해되기 위해 필요하다는 것을 밝히며 VHL과 HIF-1α의 관계를 규명하였다.

그렇다면 산소가 구체적으로 어떻게 VHL과 HIF-1α 사이의 상호작용을 조절할까? 케일린 교수와 랫클리프 교수는 산소가 정상적으로 공급되는 상황에서 VHL이 HIF-1α과 결합하는 메커니즘을 연구했다. 정상 산소 농도에서 HIF-1α에 2개의 OH기가 결합하게 되며(prolyl hydroxylation) 단백질이 변형된다. VHL은 변형된 HIF-1α와 결합해 단백질 복합체를 형성한다. 이후 프로테아좀이 HIF-1α를 분해하는 방식으로 HIF-1α의 농도를 조절한다.

 

세포가 산소 농도에 적응하는 메커니즘은 여러 질병과 연관되어 있다. 예를 들어 만성 신부전 환자들은 EPO 발현이 감소해 적혈구의 활동성이 감소하여 심각한 빈혈에 시달리기도 한다. 또한 여러 암 질환에서 산소에 의해 조절되는 기작은 혈관 생성을 촉진시키고 암 세포의 효과적인 분열을 위해 대사과정을 재설계하는 데에도 사용된다. 이에 따라 제약 학계 및 산업 분야에서는 산소 감지 메커니즘을 활성화시키거나 저하시키는 약물 개발에 심혈을 기울이고 있다.

 

스웨덴의 아스트라제네카, 미국의 피브로젠, 일본의 아스텔라스는 투석중인 만성신부전 환자의 빈혈 치료제인 록사두스타트(roxadustat)을 개발하였다. 록사두스타트는 HIF의 prolylhydroxlation에 관여하는 prolylhydroxylase의 저해제로, EPO의 활동을 저해하는 반응을 감소시킨다. 그 결과로 적혈구 생성을 촉진하고 혈액의 산소 운반 능력을 높여 빈혈을 치료한다. 록사두스타트는 경구 제제로 편리하게 투여할 수 있으며, 만성신부전 환자의 삶의 질을 높이고 사망률을 감소시켜 치료를 돕는다. 주목할만한 블록버스터 신약으로 각광받고 있는 가운데, 중국, 일본에서 시판허가를 받고 미국과 유럽에서는 임상시험을 진행 중이다.

 

국내에서도 여러 제약사들이 HIF-1α를 타겟으로 한 치료제를 개발 중에 있는데, 크리스탈지노믹스는 염증성 장질환을 치료하는 물질을 연구 중이다. 크리스탈지노믹스의 후보물질인 CG-598은 HIF-1α를 안정화시키도록 작용하여, 염증성 장질환에 걸린 환자의 장벽에 국소적으로 작용해 장벽을 복구시키는 역할을 한다. 기존의 다른 치료제들이 염증을 가라앉히는 것을 주목표로 하는 것과 차이를 가지며, 현재 휴미라 등의 TNF-α 억제제가 고가의 주사제라는 점에 비해 경쟁력을 가질 것으로 보인다. 이외에도 올리패스와 에이프로젠이 HIF-1α 억제 기전을 활용하여 항암제를 개발 중이다.

The Nobel Prize in Physiology or Medicine 2019 was awarded to William Kaelin(Harvard University), Peter Ratcliffe(Oxford University) and Gregg Semenza(Johns Hopkins University professor). The achievement of  three researchers is how cells adapt to the concentration of oxygen. Oxygen is essential fuel for animal life and directly involved in the process of  converting food into fuel. When oxygen in the air is taken into the body through breathing, red blood cells in blood supply it to tissues. If you are in an oxygen-deficient environment, if you have exercised radically, or if you have a disease, your body starts to respond to this problem. Then where  is the signal for lack of oxygen coming from?



 

EPO (Erythropoietin) is produced in the kidneys and regulates the amount of red blood cells. The number of red blood cells would increase in order to provide oxygen smoothly to tissues in hypoxia. Professor Gregg Semenza has identified specific gene that contributes to controlling this process. We also learned that this is placed right next to EPO gene and involved in hypoxia situation. Professor Peter Ratcliffe who was working on the same subject at that time also found out this is basic function of kidney cells as well as all the other cells.


Professor Gregg Semenza wanted to specify the cell components which mediate the responses in hypoxia. He cultured liver cells and discovered a protein complex that binds to gene found in a previous study and named it Hypoxia-Inducible Factor (HIF). Further studies have conducted to identify HIF and found out it consists of two proteins, HIF-1α and ARNT and also the gene that encodes HIF.

The amount of HIF-1α is inversely proportional to the concentration of oxygen. If lack of oxygen, the amount of HIF-1α increases and combines with ARNT protein in the nucleus into the HRE (Hypoxia-Regulated Element) gene to increase the emission of EPO. If there is no oxygen shortage, a small protein called Ubiquitin combines with HIF-1 α to help protease (protease compound protein, proteasome) dissolve HIF-1α quickly.

When oxygen levels are low (hypoxia), HIF-1a is protected from degradation and accumulates in the nucleus, where it associates with ARNT and binds to specific DNA sequences (HRE) in hypoxia-regulated genes (1). At normal oxygen levels, proteasome quickly decomposes HIF-1a (2). Oxygen binds to HIF-1a  in the form of OH(3), and VHL recognizes HIF-1a  and forms a complex to complete the decomposition process.

 

 

The specific mechanism of HIF-1α decomposing was discovered by William Kaelin, a professor at Harvard University who was studying genetic diseases and cancer at the time. While he was studying Von Hippel-Lindau syndrome, he found that if there is a problem with the VHL gene, there is a higher risk of developing certain cancers. The VHL gene encodes proteins that inhibit the development of cancer, and has also found that many of the cancer cells exhibit hypoxia-regulated genes when they lack the VHL gene. Other studies have shown that VHLs combine protein with Ubiquitin to cause it to break down from protease. Overall, Professor Ratcliffe and his research group have identified the relationship between VHL and HIF-1α, stating that VHLs interact with HIF-1a  and are necessary to decompose.


So how does oxygen specifically control the interaction between VHL and HIF-1α? Kaelin and Ratcliffe studied the mechanism in which VHL combines with HIF-1α in situations where oxygen is normally supplied. At normal oxygen concentrations(normoxia), two OH units are combined in HIF-1α (prolyl hydroxylation) and the protein is deformed. VHL combines with modified HIF-1α to form a protein complex. Then, the concentration of HIF-1α is adjusted by the way the proteasome disassembles HIF-1α.

The mechanism by which cells adapt to oxygen concentrations is associated with various diseases. Chronic renal failure patients, for example, may suffer from severe anemia due to reduced EPO manifestations, reducing the activity of red blood cells. In many cancer diseases, oxygen-regulated mechanisms are also used to promote blood vessel production and to redesign metabolic processes for effective division of cancer cells. Accordingly, the pharmaceutical academic and industries are keen on developing drugs that activate or degrade oxygen detection mechanisms.

 

AstraZeneca of Sweden, FibroGen of the United States and Astellas of Japan have developed roxadustat, a treatment for anemia in patients with chronic renal failure who are on dialysis. Roxadustat is an inhibitor to the prolyhydroxylase involved in the prolyhydroxlation of the HIF, which reduces response that inhibits EPO activity. As a result, it promotes the production of red blood cells and improves the ability of oxygen transport in the blood to treat anemia. Roxadustat can be conveniently administered with oral preparation, improving the quality of life for patients with chronic renal failure and helping with treatment by reducing mortality. In the spotlight as a notable blockbuster drug, it has been granted marketing licenses in China and Japan and is undergoing clinical trials in the United States and Europe.

In Korea, many pharmaceutical companies are also developing treatments targeting HIF-1α. CrystalGenomics is working on a treatment substance that can be used in inflammatory intestinal diseases. CG-598 as a candidate material for CrystalGenomics works to stabilize HIF-1α, which serves to restore the barrier by acting locally on the barrier of patients with inflammatory bowel disease. It is different from other existing treatments focusing on relieving inflammation, and is expected to be competitive compared to current TNF-α inhibitors such as Humira, which are expensive injections. In addition, OliPass and Aprogen are developing anti-cancer drugs using HIF-1α inhibitory mechanisms.
