떠오르는 뇌 기능 개선제, 니세르골린

 

 

 2023년, 뇌 기능 개선제 시장에서 핵심 품목이던 ‘콜린알포세레이트’ 성분 의약품이 급여 환수 대상으로 지정되었다. 콜린알포세레이트 성분은 뇌신경의 손상으로 저하되었던 신경전달 기능을 정상화하고, 손상된 뇌세포에 직접 작용해 신경세포의 기능을 높이는 약물로 알려져 있었다.

 

이러한 '콜린알포세레이트' 성분이 치매 외 처방 품목 급여 대상에서 제외되며, '니세르골린' 성분제가 대체제로 떠오르고 있다. 

 

니세르골린의 오리지널 의약품은 1978년에 허가를 받은 일동 제약의 ‘사미온정’이다. 5mg, 10mg, 30mg 등 3개 용량으로, 이들 중 치매 증후군의 일차적 치료와 관련된 30mg이 특히 주목받았다.

 

최근 뇌 기능 개선제로서 니세르골린 성분이 떠오른 후, 한미약품이 ‘니세르골린정’을 약제 급여목록에 이름을 올렸다. 한미약품이 10mg, 30mg 제품을 출시한 후 지난 12월 환인제약, 하나제약, 알보젠코리아 등 3개의 회사가 잇따라 허가를 받았다. 올해 2월에는 26개 업체에서 26품목이 ‘니세르골린’성분을 허가받는 등 니세르골린의 위상이 나날이 증가하고 있다.

 

니세르골린 성분은 주로 5, 10, 30mg 등으로 나뉘며, 용량에 따라 다른 적응증을 가진다. 우선 니세골린정 5, 10mg은 뇌경색 후유증에 수반되는 만성 뇌 순환 장애에 의한 의욕 저하 개선, 노인 동맥경화성 두통, 고혈압 보조요법에 사용된다. 니세골린정 30mg은 일차성 퇴행성 혈관 치매 및 복합성 치매와 연관된 치매 증후군의 일차적 치료를 담당한다. 집중력 장애, 기억력 손상, 적극성 부족, 판단력 장애 등의 목적으로 사용한다.

 

그렇다면 뇌 기능 개선제로 떠오르는 의약품인 니세르골린에 문제점은 없을까?

 

앞서 말했듯이 니세르골린은 고혈압의 보조요법에 사용되기 때문에, 이 부분에서 문제가 발생할 수 있다. 혈압강하에 따른 고혈압 보조요법의 적응증을 가지기 때문에, 이미 혈압강하제를 복용 중인 환자들에게는 각별한 주의가 필요하다. 대부분의 뇌인지 장애와 치매 환자들은 고령이기 때문에, 고혈압 제제를 이미 복용하고 있는 환자들이 많다. 이러한 이유로 대상 환자들에게 적용하기에 어려움이 존재한다.

 

그럼에도 '콜린알포세레이트' 성분 의약품을 대체한다는 점에서 다양한 이점들이 존재하기 때문에, 부족한 부분들이 개선되어 뇌 기능 개선제로서 환자들에게 많은 도움을 주는 의약품으로 자리 잡길 기대해 본다.

After the recent emergence of nicergoline as a brain function improvement drug, Hanmi Pharmaceutical listed 'nicergoline tablet' on its drug benefit list. After Hanmi Pharmaceutical released 10mg and 30mg products, three companies, Hwanin Pharmaceutical, Hana Pharmaceutical, and Albogen Korea, were approved one after another in December. In February this year, 26 items from 26 companies were approved for 'nicergoline' ingredients, and the status of nicergoline is increasing day by day.

 

Nicergoline components are mainly divided into 5, 10, and 30mg, and have different indications depending on the dose. First, Nicergoline tablets 5 and 10mg are used to improve motivation due to chronic brain circulation disorders associated with the aftereffects of cerebral infarction, arteriosclerotic headaches in the elderly, and adjuvant therapy for hypertension. Nicergoline tablet 30mg is responsible for the primary treatment of dementia syndrome associated with primary degenerative vascular dementia and complex dementia. Specific symptoms are used for the purposes of impaired concentration, memory damage, lack of activeness, and judgment.

 

Then, is there any problem with nicergoline, a drug that is emerging as a brain function improvement drug?

 

As mentioned earlier, there is an analysis that nicergoline may be a problem in that it is used for adjuvant therapy for hypertension. Since it has an indication of adjuvant therapy for hypertension according to blood pressure drop, special attention is required for patients who are already taking blood pressure drop drugs.
