2020년 노벨 생리의학상 'C형 간염 발견' 

2020년 노벨상 생리·의학상은 하비 알터(Harvey J. Alter), 마이클 호튼(Michael Houghton), 찰스 라이스(Charles M. Rice)에게 수여됐다이들은 C형 간염 바이러스를 발견함으로써 간경변과 간암의 주요 원인인 혈액 매개 간염 퇴치에 기여한 공로를 인정받았다.

 

미국국립보건원(NIH) Harvey J. Alter 교수는 1970년대 중반 수혈과 관련된 바이러스 질환을 처음 보고했다. 그 후 관찰된 이 바이러스가 C형 간염 바이러스로 알려졌다.

이어 캐나다 앨버타대학교 Michael Houghton 교수는 1989 C형 간염 바이러스의 존재를 처음으로 규명했다미국 록펠러대학교(Rockefeller University) Charles M. Rice 교수는 C형 간염 바이러스의 내부 단백질 구조를 처음 밝혀냈다.

노벨상 위원회는 "세 의학자들의 공로 덕분에 바이러스에 매우 민감도가 높은 혈액검사가 가능해졌다. 이로 인해 전 세계 여러 지역에서 수혈 후 간염을 거의 없애면서 세계인의 건강을 크게 향상시켰다" "또한, C형 간염을 겨냥한 항바이러스 약물을 급속하게 개발할 수 있도록 했다"고 밝혔다.

그럼 C형 간염이란 무엇일까? C형 간염은 C형 간염 바이러스(Hepatitis C Virus)에 의해 간세포 염증과 손상이 유발되는 간질환이다. 1~6까지 6개의 유전자형이 있으며, 아형은 70개 이상이다. 주된 전파경로는 주사기 공동사용, 수혈, 혈액투석, 성접촉, 모자간 수직 감염 등의 혈액매개이다. C형 간염 바이러스에 노출되면 급성 간염을 앓게 되는데대부분 가벼운 감기증상 또는 거의 무증상이어서 모르고 지나가는 경우가 많다. 노출된 환자의 70% 정도는 6개월 이상 체내에서 머무르며 만성 간염으로 진행하게 되며, 이중 30~40%는 간경변 및 간암으로 진행하게 된다. 매년 35~50만명에 이르는 환자들이 C형 간염바이러스와 연관된 간질환으로 사망하고 있어서 심각한 세계 보건 문제로 인식되고 있다.

이러한 C형 간염을 일으키는 C형 간염 바이러스(Hepatitis C virus, HCV)는 크기가 작고(55-65nm) 막이 있는 양성 단일가닥 RNA 바이러스로, 플라비바이러스과에 속한다. HCV는 감염 시 숙주의 면역체계를 빠져나가 HCV 특이적 중화항체가 형성되지 않으며, 돌연변이로 인한 유전적 다양성(quasi-species)이 심해, 예방을 위한 백신의 제작에 어려움이 있다. 현재 C형 간염 백신은 존재하지 않지만, 최근에는 매우 혁신적인 치료법이 개발되어 대부분의 환자들이 특별한 부작용 없이, 높은 성공률로 짧은 치료기간 내에 바이러스를 박멸할 수 있다

 

치료법의 발전에는 C형 간염 바이러스 복제를 직접 억제하는 여러 가지 바이러스 직접 작용제제(direct-acting antiviral agents, DAAs) 신약들이 대거 등장했기 때문이다. 이러한 약들의 장점은 경구용 약제들로 구성되어 더 이상 주사제를 사용하지 않고, 치료기간이 2~6개월 사이로 단축되었으며, 치료기간 중 부작용이 거의 없고, 치료 성적 또한 90% 이상 높다는 점이다. 따라서 C형 간염 바이러스는 이제 금기증이 없는 모든 환자에게 치료가 권고되고 있으며, 치료에 대한 유일한 금기증도 C형 간염 외 다른 신체 질환으로 인해 장기 생존을 기대할 수 없는 경우를 제외한 거의 대부분의 경우에 치료가 가능하게 되었다.

현재 국내 승인된 약제는 ‘아수나프레비르’(Asunaprevir, 제품명 순베프라), ‘다클라타스비르’(Daclatasvir, 제품명 다클린자), ‘소포스부비르’(sofosbuvir, 제품명 소발디), ‘레디파스비르+소보스부비르’(Ledipasvir+Sofosbuvir, 제품명 하보니) 복합제, 옴비타스비르+파라타프레비르+리토나비 복합제(Ombitasvir+paritaprevir+ritonavir, 제품명 비키라), 다사부비르(Dasabuvir, 제품명 엑스비라), 엘바스비르/그라조프레비르 복합제(elbasvir+grazoprevir, 제품명 제파티어), 글레카프레비르/피브렌타스비르 복합제(glecaprevir+pibrentasvir, 제품명 마비렛)이 있다. 이 외에도 소포스부비르/벨파타스비르 복합제(sofosbuvir+velpatasvir, 제품명 엡클루사), 소포스부비르/벨파타스비르/복실라프레비르 복합제(sofosbuvir+velpatasvir+voxilaprevir, 제품명 보세비)도 있지만 국내 허가를 받지 않았다.

 

The Nobel Prize in physiology or medicine 2020 'for the discovery of Hepatitis C virus'

The 2020 Nobel Prize in Physiology and Medicine was awarded to Harvey J. Alter, Michael Houghton and Charles Rice. They were credited with helping fight blood-mediated hepatitis, a major cause of liver cirrhosis and liver cancer, by discovering hepatitis C virus.

Harvey J. Alter, a professor at the National Institutes of Health (NIH), first reported viral diseases related to transfusion in the mid-1970s. The virus, which was observed, is known as the hepatitis C virus.

Michael Houghton, a professor at the University of Alberta in Canada, then first identified the existence of hepatitis C virus in 1989. Professor Charles M. Rice of Rockefeller University in the United States first uncovered the internal protein structure of the hepatitis C virus.

The Nobel committee said, "Thanks to the contributions of the three medical scientists, blood tests that are highly virus-sensitive have become possible.” “This has greatly improved the health of people around the world by almost eliminating hepatitis after transfusion in many parts of the world," he said. "It also allows us to rapidly develop anti-virus drugs targeting hepatitis C."

So what is hepatitis C? Hepatitis C is a liver disease caused by the hepatitis C virus, which causes liver cell inflammation and damage. There are six genotypes from 1 to 6, and more than 70 subtypes. The main propagation pathways are blood mediums such as co-use of syringes, transfusion, hemodialysis, sexual contact, and maternal vertical transmission. Exposure to hepatitis C virus causes acute hepatitis, most of which are mild or almost asymptomatic and often pass unknowingly. About 70 percent of exposed patients will stay in the body for more than six months and proceed with chronic hepatitis, 30 to 40 percent of whom will be treated with liver cirrhosis and liver cancer. Between 350,000 and 500,000 patients die of hepatitis C-related liver disease every year, which is perceived as a serious global health problem.

The Hepatitis C virus (HCV), which causes this hepatitis C, is a benign single-strand RNA virus with a small size (55-65 nm) and belongs to the Flavivirus family. HCV escapes the host's immune system in case of infection and does not form HCV-specific neutralizing antibodies, and the genetic diversity caused by mutations is severe, making it difficult to produce vaccines for prevention. Currently, no hepatitis C vaccine exists, but recently a very innovative treatment has been developed, allowing most patients to exterminate the virus in a short period of treatment with a high success rate, without any special side effects.

The development of the treatment method is due to the emergence of a number of new virus direct-acting antiviral agents(DAAs) that directly suppress the replication of hepatitis C virus. The advantages of these drugs are that they are oral medications, so no more injections are used, the duration of treatment has been shortened to two to six months, have few side effects during the treatment period, and the performance of treatment is more than 90 percent higher. Thus, the hepatitis C virus is now recommended for treatment in all patients without a taboo, and the only taboo for treatment has become treatable in almost most cases, except that long-term survival cannot be expected due to other physical ailments other than hepatitis C.

The currently approved drugs in Korea are Asunaprevir(product name: Sunvepra), Daclatasvir (product name: Daklinza), Sofosbuvir(product name: Sovaldi), and Ledipasvir+Sofosbuvir(product name:Harvoni), Ombitasvir+paritaprevir+ritonavira(product name:Viekirax), Dasabuvir(product name:Exviera), elbasvir+grazoprevir(product name:Zepatier), glecaprevir+pibrentasvir(product name:Maviret). In addition, the Sofosbuvir/Velpatasvir(product name: Epclusar), sofosbuvir+velpatasvir+voxilaprevir(product name:Vosevi) are not licensed in Korea.
