한국과 일본에서의 주요 제약산업 현황


 


1.   주요 의약품 (한국)


 

의약품(Drug)의 종류는 개발 방법에 따라 크게 ‘합성의약품(Chemical Drug), ‘바이오의약품(biologics)으로 나눌 수 있습니다. 합성의약품은 화학물질을 합성하여 만든 것이고, 바이오의약품은 살아있는 생물에서 뽑아낸 재료로 약을 만듭니다. 의약품의 종류는 또한 신약(Original Drug)과 복제의약품으로 나눌 수 있는데, 합성의약품의 복제의약품은 ‘제네릭(generic)이라는 이름으로 부르는 반면 바이오의약품의 복제의약품은 ‘바이오시밀러(bio-similar)라고 부릅니다. 그 이유는 합성의약품은 합성물질이나 제조방법을 원래의 제품과 다르게 하더라도 최종 결과물만 같으면 생물학적으로 동등한 효과를 낼 수 있는 반면, 바이오의약품은 대장균이나 효모, 동물 세포 등 살아있는 세포에서 단백질을 뽑아내고 생산하기 때문에 제조과정에 따라 최종 산물이 다를 수 있고, 심지어는 동일한 제조과정을 거쳐도 누가 언제 만들었느냐에 따라 서로 다른 제품이 나오기도 하기 때문입니다. 따라서 바이오의약품의 복제의약품은 바이오신약(Original biologics)과 완전히 동일한 의약품을 만드는 것이 현실적으로 불가능하기 때문에, '시밀러(similar)'라고 부르는 것입니다.


 

한국의 제약회사는 신약개발보다는 주로 복제의약품을 개발하고 판매합니다. 그 중에서도 한국의 바이오시밀러는 기술적 강점을 배경으로 초기 진입에 성공해 글로벌 시장을 선도하고 있습니다. 바이오시밀러 산업에서 떠오르는 회사 중 하나는 ‘셀트리온(CELLTRION)입니다. 셀트리온은 2세대 바이오의약품인 항체의약품을 만드는 회사로, ‘항체 바이오시밀러’라는 신시장을 만들어냈습니다. 셀트리온이 개발한 대표적 바이오시밀러로는 ‘램시마주’가 있습니다. 램시마주는 국산 바이오시밀러 류마티스관절염 항체치료제로, 염증에 관여하는 TNF(종양괴사인자, Tumor Necrosis Factor)-alpha를 타깃(target)으로 한 면역억제제로서 작용합니다. 램시마주는 2015년 식품의약품통계연보에 따르면 2477만달러를 올려 유전자재조합의약품 수출 상위 10위권 가운데 최고액을 기록했습니다.

1.   주요 의약품 (일본)

 

 일본은 초고령화 시대로 진입해 있습니다. 전문가들은 2060년에는 일본의 총인구는 9천만명에 진입할 예정이고 그중 40%가 고령화층에 육박할거라 예측하고 있습니다. 또한, 암에 의한 사망률 또한 꾸준히 증가할 거라 예측하고 있습니다. 2015년의 한 조사에 따르면 1985년에 비해서 암에 의한 사망률이 2배로 증가하였습니다. 따라서, 암 조직에 대한 약의 필요성은 점점 더 중요하게 대두되고 있으며, 암에 대한 항체(표적항암제)가 주된 치료제로 각광받고 있습니다.

 

표적항암제란 무엇인가?

 

표적항암제는 항체를 사용하여 암세포와 같은 세포의 표면에 있는 표지를 인식하여 우리몸에 항원으로 작용하는 암세포를 제거하는 역할을 합니다. 따라서, 특정 암세포에 작용하기 때문에 높은 치료 효과를 가지게 되고, 방사선 치료 같이 비특이적 작용 항암제에 비해서 부작용이 적습니다. 일부 표적항암제는 조직에서 과발현된 암조직에 대한 항체로 약으로서 생산이 됩니다.  





표적항암제 연구


 


 표적항암제를 효율적으로 생산하기 위해서(저비용, 대량생산) 전임상과정에서 동물세포와, 대장균 그리고 효모, 식물을 이용하여 일본에서는 각 연구실에서 연구가 활발히 진행되고 있습니다. 현재는, 서로 다른 종류의 표적항암제에 대한 연구가 진행중에 있습니다. 기존의 표적항암제는 제네릭 기술을 활용하여 만듭니다. 하지만, 최근에는 여러 번 결합이 가능하는 사이클링 표적항암제(Cycling antibodies)가 활발히 연구가 진행되고 있습니다. 그리고 표적항암제의 더 높은 효율을 가지며 더 다양한 기능을 가지는 표적항암제를 개발하기 위한 연구가 진행중에 있습니다.


  


2.   R&D 투자(한국)


R&D, Research and Development’의 약자로, 기업에서 연구결과를 바탕으로 하여 상품을 개발하는 활동을 의미합니다. 한국에서 연구개발 투자 성과 상위권 제약사로는, '한미약품', '동아에스티', 'LG 생명화학',’셀트리온’,’녹십자등이 있습니다.


 

국내 R&D투자는 바이오 제약기업들의 신약, 바이오 시밀러 등을 새로운 성장동력으로 삼아 연구 투자를 진행하고 있습니다. 특히나, 한미 제약회사는 회사 초기부터 적극적인 투자를 통해서 한국의 R&D분야를 이끌어 나가고 있습니다. 2015년에는 7천억원의 기술 수출계약을 일라이릴리와 진행하는 등 글로벌 신약개발 투자를 진행하고 있습니다. 또한 가장 최신 조사에 따르면 바이오시밀러 분야의 선두인 셀트리온이 매출액 대비 가장 높은 R&D비중을 보여주며, 한국의 바이오 산업을 이끌어 나가고 있습니다. 









2.R&D 투자(일본)


 

세계적인 TOP 50 제약회사 순위에서 10개 이상의 제약회사가 포함되어 있을 정도로, 신약에 대한 투자가 활발히 진행되고 있으며 제약산업 이외의 산업에도 많은 투자가 이루어지고 있습니다. 신약 개발에 적게는 수백억에서 조단위의 돈을 투자하고 있습니다. 일본 10대 매출 기업의 총 R&D평균 지출은 2010년 약 100억에서 2015년 약 11조 규모로 증가했습니다.

비록 수십억의 돈과 많은 시간이 신약개발에 소모가 되고, R&D지출에서 제약 산업(신약개발)에 쓰이는 돈은 제일 많은 비중을 차지합니다. 일본 제약 제조협회의 조사(Japan Pharmaceuticals Manufacturers Association datebook)에 따르면 제약 제조업에 대한 투자는 광업, 석기 산업, 축산업의 36배에 이른다고 보고했습니다.






 


 


 

3.  제약회사의 구체적인 부서(한국)


 


한국 제약회사는 주로 크게 다섯 가지 부서로 나누어져 업무가 이루어집니다.


 


1)영업, 마케팅(product marketing & sales)


2)연구/개발(Research & Development)


3)임상 (CRA)


4)생산,품질 관리(QC & QA)


5)재정/관리/홍보


 


 


1)영업, 마케팅(product marketing & sales)


 

영업은 제약회사의 꽃이라고도 불리며, 회사의 이윤이 영업매출에서 오기 때문에 제약회사의


모든 전략은 영업을 중심으로 돌아갑니다. 제약회사에서 영업을 담당하는 사람들을 MR(Medical Representative), 의약 정보전달자라고 하며 주요 업무는 질환, 제품에 대한 학습을 기반으로 정확한 의약 정보를 고객에게 전달하는 역할을 합니다. 이를 통해 고객인 의사의 진료 만족도를 높이고, 환자에게는 치료의 만족도를 높일 수 있으며 궁극적으로 기업의 성과 창출에 직접적으로 기여합니다. 과거에는 비약대 출신들이 주로 영업 업무를 담당하였지만, 요즈음에는 약대 출신 MR들도 많이 찾아볼 수 있습니다.


 


마케팅 팀은 영업사원들이 시장에 나가서 제품을 설명하기에 앞서 제품별 판매 전략을 세워주는 역할을 맡습니다. 제약회사는 제품마다 매니저를 두고 있고, PM(product manager)"이라고 부릅니다. PM은 담당 제품과 관련된 시장 상황 등을 분석하여 제품의 성공적인 판매를 위한 이벤트를 기획하고 실행하는 업무를 담당합니다. 마케팅 업무를 위해서는 영업에 대해 잘 알고 있어야 합니다.


 


 


2)연구/개발(Research & Development)


 


연구


연구부는 연구소에서 여러 질병에 대한 새로운 target을 발견하고, 좋은 약효를 나타내는 물질/성분을 개발하는 업무를 담당합니다. 연구 분야는 제약회사의 연구소는 화학, 화학공학, 생물학, 약학 등의 관련 전공을 우대하여 뽑고, 보통 석사 이상의 학력이 요구 되기도 합니다.


 


개발


개발팀은 신약을 개발하는 과정부터 의약품으로 허가 및 등록한 후 출시되기까지의 모든 과정을 수행합니다. 의약품은 그 특성상 환자의 목숨과 바로 연결되기 때문에 식품의약품안전처(이하 식약처)에서 정한 철저한 규정 속에서 생산 및 판매되고 있고, 이런 철저한 규정을 하나하나 챙기고 관리하는 조직이 바로 개발팀입니다. 개발팀의 직무는 다시 RA(Regulatory Affairs), 학술, 사업/허가개발, 약가 팀으로 세분화되어 이루어집니다.


 


- RARegulatory Affairs의 약자로, 말 그대로 '규정과 허가'와 관련된 일을 함으로서 개발 부서의 주된 업무를 담당합니다. 동물실험(비임상실험), 임상 실험에 적용되는 규제는 물론이고 약이 허가를 받기 위해 충족시켜야 하는 많은 조건과 자료들을 일일이 챙겨서 제출하고, 결과를 식약처와 조율하는 역할을 한다. 의약품은 연구부터 출시 및 시판 후까지 많은 법과 규정에 의해 관리되고 있고, 따라서 RA의 역할은 매우 넓고 중요합니다.


 


- MSL(Medical Science Liaison)은 학술팀 소속으로 주요 업무는 소속된 회사에서 개발된 새로운 의약품이나 의료 기기에 대한 정보를 연구자와 의료인들에게 전달하고, 마케팅이 상용화할 학술적 데이터를 확보하여 전달합니다. 의료인과 연구자들, 회사 담당자 등 다양한 사람과 함께 소통하며, 특히 마케팅 팀과 긴밀한 관계를 맺고 있습니다. 마케팅 팀이 만든 프로모션 자료를 검토하고 필요한 논문 등의 자료를 제공하며 MR, PM 직원들의 제품/질환 관련 교육을 담당합니다. 따라서 MR보다 제품 및 경쟁품 그리고 질환과 관련된 최신 연구 동향 등에 관해 더욱 전문적인 지식을 가져야하고, 마케팅의 역할에 대한 이해가 필수적입니다. 최근에는 제품 심포지엄과 같은 학술행사와 제품 임상시험을 기획하고, 제안합니다. 우리나라의 경우, MSL은 영업, 마케팅, 학술 팀이 중첩된 역할을 하고, 주로 약사 위주로 구성되어 있습니다.


 


사업/허가개발(Business development)


 


-BD(Business development)질환, 약물, 경쟁동향 등 국내외 의약품 개발 관련 정보를 수집해 분석하여 회사의 사업전략 및 제품개발 계획을 세워 실행하는 업무를 담당합니다. 파트너사를 타겟팅하고 함께 협력하여 성공적인 신제품 개발에 기여합니다.


 


-약가팀은 건강보험공단/심평원과 약가를 협상하는 부서로 약물경제성 평가 자료를 작성 및 제출하고 약가가 결정되게 합니다. 신약이 최단기간에 높은 약가를 획득할 수 있도록 하며 개량신약 및 제네릭의 경우 정확한 약가 산정이 될 수 있도록 합니다.


 


3)임상(clinical research)


임상팀은 세포와 동물에서 약효와 독성 확인된 신약후보물질을 가지고, 사람에서 그 유효성과 안전성을 확인하는 단계를 담당합니다. 임상시험 관련 직무도 여러 부서로 나누어져 있습니다.


 


- 'CRA(clinical research associate)'는 임상시험담당자를 말하고, 제약회사와 임상시험 대행기관인 CRO(Contract Research Organization)회사에서 근무합니다. CRA는 임상시험 모니터링 요원으로서 연구 프로토콜에 맞게 임상시험이 진행되는지 직접 병원에 가서 체크 하고 환자 등록을 독려하는 역할을 합니다.


 


- DM(Data Management)'은 임상에서 나온 테이터를 통계 분석하는 부서입니다.


 


- PV(Pharmacovigiliance)는 신약후보물질 단계부터 임상시험 그리고 허가 후까지 일어나는 약물 유해반응과 같은 의약품의 안전성 정보를 모니터링하고, 수집하여 분석하는 업무를 담당합니다. 의약품 안전성에 문제가 생길 시 KIDS(한국의약품안전관리원)나 식약처에 보고하여 소통하는 역할을 합니다.


 


4)생산/품질관리(Production Management/Quality Management)


 


생산관리는 보통 제약회사의 공장에서 현장 감독 역할로, 공장의 약품 생산라인 관리, 약품 생산 계획 수립, 그리고 직원들의 교육 등을 담당합니다.


 


품질관리 업무에는 QC(Quality Control)QA(Quality Assurance)가 있는데 각각 품질관리, 품질보증을 담당합니다. QC는 이화학, 미생물, 기기 분석 시험 등을 통해 의약품을 제조하는데 필요한 원자재 그리고 제품이 환자에게 공급되기까지의 전 제조 공정과 품질을 평가합니다. 생산된 완제품에 대한 품질 평가도 QC가 담당합니다.


QA는 주로 문서 작업을 통해 제품의 제조부터 최종적으로 출하할 때까지 모든 문서와 규정, 절차를 관리합니다. QA의 최종 승인이 나야 제품을 출하할 수 있습니다. 생산, 품질 관리부서는 부서의 특성상 공장에 있어야 하므로 대부분 지방근무를 합니다.


 


 


3.제약회사의 구체적인 부서(일본)


일본 또한 한국처럼 제약회사 내에 세부 부서들이 있습니다. 이 부서들은 신약개발을 위해서 업무 종류에 따라 나뉘어 있습니다. 크게는 7가지 부서로 이루어져 있습니다.

3)   관리 약사(QA)

1)연구 개발팀 Research & Development (R&D)


연구팀


크게 연구팀은 새로운 신약을 개발하는 역할을 담당하고 있습니다. 유효물질에 대한 연구를 수행하며 합성하는 역할을 수행합니다. 특히나 이 유효물질은 약으로서 역할을 수행할 수 있도록 만들어 집니다. 약으로써의 효과도 중요하지만 약의 안전성과 보존 또한 신약개발에 있어서 주요한 요소로 작용하게 됩니다. 신약개발에는 많은 시간과 큰 비용이 지출되는 만큼 연구팀은 제약회사에서 핵심적인 역할을 수행하고 있습니다.


개발팀


개발팀은 임상과정에서 사람들에게 약을 투여함으로써 안전성과 유효성을 확인하는 역할을 수행하고 있습니다. 병원과 특정 제약 부서에서 임상을 수행하고 있습니다. 이 부서에 일하기 위해서는, 약학에 대한 높은 수준의 지식을 요구하며 전문성 또한 요구하고 있습니다.


 


2)임상수행원(CRA)


CRA는 한국에서의 역할과 비슷한 역할을 수행하고 있습니다. 전반적인 임상실험을 돕는 역할을 하고 있으며, 임상간호사와 임상을 수행하는 의사를 도와 임상을 진행하고 있습니다. 특히나, CRA는 임상 관련된 부서들과 긴밀한 소통을 하며, 많은 사람들과 접촉하여 임상진행에 주된 일원으로서 역할을 수행하고 있습니다.  


 


3)관리 약사


관리 감독 약사는 한국에서의 QA와 비슷한 역할을 수행하고 있습니다. 의약품의 품질을 위해서 공장 시설에서 전반적인 관리감독을 수행하고 있으며, 생산된 약에 대한 불만(complain)과 관련된 행정적인 업무를 담당하여 제약회사의 약에 대한 책임을 지고 있습니다..


 


4)제약 영업(MR)


일본에서의 제약회사 영업은 한국에서의 역할과 비슷하다. 의사들에게 약에 대해서 설명하고 병원에서 회사의 약을 사용도록 권유하는 역할을 합니다. 또한 환자들이 약을 안전하고 올바르게 사용하도록 약에 대한 약학 지식을 제공하는 역할도 수행하고 있습니다. 영업사원들은 제약회사의 약을 판매하기 위해서, 제약회사의 전반적인 시스템을 숙지해야 하며 약에 대한 지식과 더불어 의료품과 윤리에 대한 기술적 지식 또한 잘 알고 있어야 합니다. 일부 회사에서는 제약 영업사원들을 위해서 이러한 기술을 가르치기 위해서 MR 교육을 실시합니다.


5) 학술팀(DI)


학술팀의 역할은 약사들과 의사 그리고 같은 회사 영업사원들에게 약학 정보를 제공하는 역할을 수행하고 있습니다. 안전한 약의 사용과 문헌정보를 제공하고 있습니다. 학술팀은 약학 정보 문의에 대해 대답해주는 역할을 수행하기 때문에 약에 대한 최근 지식을 숙지해야 합니다. 그리고 때로는 그들은 연구기관의 정보를 제공하는 역할도 합니다. 약학 문헌을 읽을 수 있어야 하고, 약학 관련된 용어들도 숙지해야 하기때문에 일정 수준 이상의 영어 수준을 요구합니다.


6)회사 내 병원에서의 약사


몇몇 주요 회사들은 회사내에 클리닉을 보유하고 있으며 회사내의 병원에서의 약사는 직원들의 건강을 유지하기 위해 노력하고 있습니다.  


7)그 외 약학 관련된 일


의약품, 의료 장비, OTC의약품, 화장품 및 유사 의약품 등 새로운 의약품에 대한 제약 업무를 신청하는 것이 그들의 주된 업무로, 자신의 회사에서 데이터를 수집하고 관련 회사에서 데이터를 번역을 수행하고 있습니다. 그들은 서류 작성에 있어서 많은 법과 규정을 따라 작성하며 약학 지식을 요구하고 많은 서류 업무 작문 실력을 요구하기도 합니다. 또한, 사업 수준과 기술적인 단어 기술에서 영어 능력을 필요로 하기도 합니다.


 


 


 


 


4.기타 제약회사 (한국)- 북한의 제약회사


 


북한의 제약회사


한국과 일본의 제약 산업을 비교하던 와중에, 남북정상회담이 진행되는 것을 보고 한국 뿐만 아니라 일본의 약학대학생들도 북한의 제약 산업에 대한 궁금증을 가지고 있을 것이라는 생각이 듭니다.


 


북한의 제약회사는 평스제약합영회사(이하 평스제약)가 유일합니다. 평스제약은 북한 보건복지부 산하 평양제약사와 스위스 기업이 포함된 북부발전제약컨소시움의 합작으로 20026월 설립된 회사로 북한에서 진통제와 항생제 등 기초의약품을 생산, 판매하고 있습니다.


 


평스제약은 총 51개의 전문·일반약·건기식을 보유하고 있으며, 연간 정제 18700만개와 캡슐 5700만개를 생산하고 있습니다. 주로 항생제인 아목시실린, 클로람페니콜, 시플로플록사신, 독시사이클린, 에리스로마이신, 테트라사이클린, 코트리목사졸, 항원충제 메트로니다졸을 만들며, 대표적 해열소염진통제 중 하나인 아스피린과 파라세타몰과 이를 바탕으로 평스아스피린, 평스파라세타몰을 만든 것으로도 확인됩니다. 특히 지난해 판문점에서 탈북한 북한 병사 몸에서 회충이 발견되며 북한 지역 위생 환경이 논란이 된 가운데 평스제약도 알벤다졸과 메벤다졸 성분 구충제를 생산하고 있습니다.


 


또한 북한 전역에 약국과 도매선을 가지고 있으며 창고 보관과 유통, 계약·제조, 마케팅 등 북한 진출을 위한 투자 서비스를 제공합니다.



위의 지도에 나타난 바와 같이 주로 평양과 평성에 11개의 약국 체인을 운영하고 있으며, 2018년에는 남포, 원산, 함흥의 세 곳에 추가로 약국이 설립할 계획이라고 발표했습니다. 평양에 위치한 약국 중 김일성 주석과 김정일 국방위원장의 동상에서 가장 가까운 약국은 매일 24시간 영업을 한다는 것이 특징적입니다.

 

평스제약은 2007년 제조공장에 대한 GMP 인증을 받았고, 국제적십자사 등 인도적 지원 기관들은 이를 통해 북한에 의약품을 공급하고 있습니다.

현재는 유엔 안보리 대북제재 결의 2375호에 의해 북한과의 합작 사업 설립, 유지, 운영이 전면 금지됨에 따라 기존 합작 사업체는 폐쇄된 상태입니다. 평스제약에서는 기초의약품 공급 부족으로 일반 주민이 고통을 겪을 수 있다는 이유로 제재 대상에서 빼달라고 요청한 바 있지만 받아들여지지 않았으며, 이 사례는 일반 북한 주민들이 고통을 겪지 않게 하면서도 북한 정권을 압박해야 하는 국제사회 지도자들의 고민을 보여주고 있습니다.





 

4.기타 제약회사 (일본)- 칸포(일본식 생약) 중점 회사



일본에는 몇몇의 칸포 중심의 회사가 있습니다. 츠무라(Tsumura)제약회사의 칸포는 일본 칸포 시장의 85%를 차지하고 있습니다. 95.4%는 칸포 형태로 처방이 나가며, 2.3% OTC 형태로 처방이 나가고 있습니다. 칸포는 중국의 생약의 기술에 기반이 되었지만 일본식으로 많이 변화하였습니다.

년 조사에 따르면 현재 89.0%의 의사는 칸포를 처방하고 있습니다. 2008년 조사에 비해 5.5%증가한 수치입니다. 50%이상의 의사들이 "칸포는 높은 치료 효과를 가지고 있다"고 대답할 정도로 칸포의 효능을 인정하고 있으며, 그 이유는 서양 의학에서 효과가 없었지만 칸포를 사용하고 의학적 효과를 본 경우가 많았기 때문입니다.

 

한약의 안전한 보관법

일본시장에서는 중국산( 80%), 국산(일본) ( 15%), 라오스산( 5%)이 칸포의 원료로써 쓰이고 있습니다.  안전하고 안정적인 한약을 확보하기 위해, Tsumura는 일본 내외로 한약 재배지를 넓히고 품질과 저장 능력을 강화하고 있습니다.

 

칸포 상품의 상업성


칸포 제품의 경우 이미 1800여년 전부터 조성과 제조 과정이 잘 알려져 있습니다. 특허가 없기 때문에 누구나 자유롭게 생산할 수 있습니다. 하지만, 칸포 제품 사업에는 해결해야 할 문제가 있다. 그 중 하나는 칸포가 생물학적으로 기존 생산물과 동등하다는 것을 증명할 필요가 있습니다.  또한, 칸포는 여러 성분들로 이루어져 있기 때문에 그것을 단일 성분 제품으로 만들기 어렵습니다. 게다가 칸포는 일정 수준의 조건하에 보관되야 한다는 한계가 있습니다. 그래서, 아직까지 제네릭으로 채택된 칸포 제품은 아직 없습니다.

1.Principle drugs(Korea)


Depending on the method of development, the type of drug can be divided into 'Chemical drug' and 'biologic'. 'Chemical drug' is made from synthetic chemicals, and 'biologics' are made from materials derived from living organisms. Types of drugs can also be divided into original drug and copied drug, whereas copied drugs of chemical drugs are called 'generic drugs', copied drugs of biologic are called 'bio-similar'. The reason for this is that chemical drug is a product that can produce biologically equivalent effects if the final product is the same, even if the synthetic substance or manufacturing method differs from the original product but, the final product of biologic may vary depending on the manufacturing process, and by even 'when it has been produced and who produced it' since biologic is produced by extracting and producing proteins from living cells, such as E. coli, yeast, and animal cells. Thus, the copied drug of biologic is practically impossible to be produced as exactly same drug as the original biologics, so it is called the 'similar'.


  Korean pharmaceutical companies develop and sell mainly copied drugs rather than original drugs. Among them, Korea's bio-similar has been leading the global market with its early entry into the world based on its technological strengths. One of the companies that is emerging in the bio-similar industry is the 'CELLTRION'. Celltrion is a company that produces the second generation of biologic, an antibody bio-similar, and has created a new market. A representative bio-similar developed by Celltrion is 'Remsima Inj.'. Remsima Inj. is a local bio-similar, rheumatoid arthritis antibody treatment that targets TNF (Tumor Necrosis Factor) - alpha, which is involved in inflammation. 1.   Principle drugs(Japan)


 Japan is facing a phase of super-aging population and declining population. It is estimated that total Japanese population become less than 90 million in 2060, and aging rate get close to 40%. The mortality rate caused by cancer have increased in both sexes. The mortality rate caused by cancer in 2015 is twice as high as the one in 1985. It seems that the main reason is super-aging population. Nowadays, antibody medicines have attracted attention as medicines foe cancer.

 It is difficult to evaluate the innovation of the medications in Japan due to the medical cost reduction effect (*1) with the super-aging population.

(*1) Generic drugs, specially expanded recalculation, optimum usage guideline, trial introduction of cost effectiveness.




What the antibody medicines are


 Antibody medicines are medicines using antibodies. Antibody medicines act on antigens which are the markers on the surfaces of cells such as cancer cells. Therefore, it is expected to make high therapeutic effects and reduce side effects.

Some antibody medicines recognize the overproduced proteins made in diseased tissues as antigens.


Research on antibody medicines


 Antibody medicines research is conducted to produce them inexpensive and massive from the cost of production. Research on making antibody medicines from animal cells, E.coli and yeast, plants such as corn and soybean, eggs, cow milk, etc. are underway.


Next generation antibody medicines


 At present, research on different type of antibody medicines is conducted. Ordinary antibodies are made from genetic engineering techniques. However, in case of the new ones, for example, there are cycling antibodies that can bind multiple times over and over and are antibodies can bind to two kinds of antigens, and so on. 2.   R & D expenditure(Japan)


Increasing research funding


In Japan, there are pharmaceutical products that cost from multibillion yen to one hundred billion yen to develop one new medicine. The average R & D expenditure of the top ten sales companies in Japan was 1.726 trillion yen in 2010 and it increased to 1.4577 billion yen in 2015. The cost is twice as much money as in 2000 (746.2 billion yen)

Although it costs hundreds of millions of yen and takes long years for development, the ratio of R&D expenses to sales of pharmaceutical manufacturing industry is the highest. According to the Japan Pharmaceuticals Manufacturers Association datebook, the ration of pharmaceutical manufacturing industry is about 36 times higher than mining enterprises, stone-quarrying enterprises and gravel-gathering enterprises among 36 industries.

1)    Product marketing & sales


MR(Medical Representative) department is called The flower of pharmaceutical company, and the establishment of strategies depend on sales since the profit of company comes from sales revenue. So MR, people in charge of sales, directly contribute to the performance of pharmaceutical companies. Their main tasks are to provide accurate medicinal information to their customers after they learn knowledges related to diseases, medicines, and their rival products. They ultimately improve the satisfaction of their customers(doctors, and patients)


 

Marketing department is in charge of establishing sales strategy for each product before MR explains their products in market. Pharmaceutical companies give a mission to promote individual products to managers called PM(product manager). PM analyzes the market related to the products and is responsible for planning and executing the event for the successful sales of the products. For example, they make promotional materials for MR, and also play a part in advertising with PR(Public Relations) team. Thus, they should also know well about the work of MR.



2)    Research & Development


The Research department find new targets for several diseases and develope new substances and ingredients(candidate materials) which have medicinal effects. Research areas include various subjects such as biopharmaceuticals, natural products, and drug formulation. People major in chemistry, chemical engineering, biology, pharmacy and other related subjects are working in pharmaceutical research institutes. It usually requires a master's degree or higher and English ability.



Development department performs all the processes from drug discovery to


approval and release of new medicine. Since medicine is directly linked to patient's life, its production and sales are carried out according to strict regulations enacted by Ministry of Food and Drug Safety. So, it is the development team that administers every single regulation. Their detailed duties are divided into RA(Regulatory Affairs), MSL(Medical Science Liaison), BD(Business Development), and Pricing


 

- RA(Regulatory affairs) is the main position of development part and they deals with matters related to regulations and approval. They submit documents about the regulations that apply to preclinical and clinical study, and the requirements that are essential for the license of medicine. Also, they coordinate the results with the Ministry of Food and Drug Safety.


- MSL(Medical Science Liaison), also called as academic team, main delivers information about newly developed medicine, medical devices to researchers, medical person. Also, they deliver academic data to be commercialized to marketing team . Especially, they work closely with marketing and sales department, They review promotional materials created by marketing team, and


educate the MR, PM staff about the products and diseases by providing necessary materials such as research paper. Therefore, it is necessary to have more specialized knowledge about the latest research trends related to medicine, competitors and diseases rather than MR and to understand the role of marketing team. Recently, they plan and propose academic events and clinical trials. In case of Korea, MSLs role overlaps sales&marketing department and academic team, and it is mainly composed of pharmacists.


 


Business development


- BD(Business development) department collects and analyzes information related to domestic and international pharmaceutical drug development such as diseases, medicines, and market trends. Finally, they establishes business strategy of company and products development plan, also executes it. They work together with their partner companies and contribute fully to new product development.



- Pricing team is a department that negotiates the price of medicine with the National Health Insurance Service/Health Insurance Review&Assessment Service.They write and submit the data on drug economy evaluation and try to get a better price of medicine within short time.


3)    Clinical Research


'CRA(clinical research associate)' refers to the persons in charge of the clinical trial, and they work for pharmaceutical company or a contract research organization (CRO) company. As a clinical trial monitoring staff, they visit the clinic to check whether each steps are in accordance with the protocol. Also, they encourage patient registration.


DM(Data Management)' is the position that performs statistical analysis of the data from the clinical trials.


PV(Pharmacovigiliance) position is in charge of monitoring, collecting and analyzing the safety information of medicine such as adverse drug reactions that occur during all the steps from the candidate substance phase to clinical

trials and the product release. They report problems with the safety of medicines to the KIDS(Korea National Health Safety Administration) or Ministry of Food and Drug Safety. They communicate with those organizations continuously.


    4)    Production Management/Quality Management)


Production Management department is the on-site supervisor of pharmaceutical company factory. They are responsible for managing the production line of the factory, establishing the medicine production plan, and educating the employees.


Quality Management department is divided into two position: QC(Quality Control), and QA(Quality Assurance)


- QC(Quality Control) position assesses the raw materials of medicines, manufacturing processes and quality of medicines through physicochemical, microbiological, and instrumental analysis tests, before products are delivered to patients.


- QA(Quality Assurance) position mainly manages all the regulations and procedures from the manufacture of products to the final shipment through document works. The final approval of QA is required to ship the product.

Both Production management and Quality management department work locally because they are in the factory



3.   Department of pharmaceutical company(Japan)

 

There are several departments in pharmaceutical company. And there we divide the work to focus on develop medicines.

1)    Research and DevelopmentR&DDepartment

R&D consists of a research department and a development one, and they are in charge of different works.


A)    Research…They develop new medicines. They study and synthesize substances that have the function of purpose. Then, they research to make substances availability as medicines. They consider not only the effect but also safety and preservation of those medicines. Through these process, they develop all new medicines that we cannot find ever. In Japan, it takes long time and costs a great deal of money to complete a medicine. Therefore, especially, researchers are mostly needed by major company.


B)    Development…They dose new medicines that research department made for human body and check the safety and capability. They check the effect of medicines by doing clinical research that is important for medical development. The clinical research is done on the basis of examination and inspection with specialists in medical and pharmaceutical department. If you want to work here, you must have high-leveled skill as a pharmacist, professionality, and the language skills.


2)    Clinical research coordinator

 The purpose is to support the overall clinical exam. They ask medical institution to host a new clinical research or help doctors and patients during the terms of clinical research. In addition, office works are also their tasks. They keep in touch with the departments related to clinical research, and fix the research schedule. They work with many people (patients of its research, pharmacists and experts, doctors are related to this department,). Therefore, high communication skills are required.


3)    Supervisor pharmacists

 They are always in company to deal with administrative problems such as supervisory authorities and control medicine’s quality. They take care of the medicines as they take responsibility for the medicines.


4)    MR

 They explain about the medicines to doctors and ask doctors to use the medicines of their company in the hospital. By that, they make a contribution to help patients use medicines safely and right. And they also sell them to pharmaceutical company that makes medicines. In addition to technical knowledge of medical supplies and ethic, several skills are needed. (talking, communicating) Some companies hold MR training to teach those skills.  You can get a license of MR after that training. 


5)    DI

 The work is to inform pharmacists, doctors, MRs in the same company and general people about the medical supplies of safety and of literatures. As they have to answer the questions from doctors and MRs who go to a pharmacy to sell them, they need to get exact latest information. And sometimes they offer it to research institution. In some companies, it’s important to get technical vocabularies and speak English in business level for reading English literatures.


6)    Pharmaceutical affairs

 It’s their work to apply for pharmaceutical affairs of new medicines including medical supplies, medical equipment, OTC medicines, cosmetics and quasi-drugs Their main job is to deal with documents such as collecting data from their own company or related companies, translating application data, and making attached data. They must write documents following to law and rules. Therefore, it’s necessary to get a great deal of knowledge and writing skills. Then, they often apply for pharmaceutical affairs in English. That’s why they need English skills in business level and technical words skills.


7)    Pharmacists in company clinic

 The purpose is to keep employees’ health. Some major companies have clinics in the company and pharmacists dispense there.


4.Pharmaceutical company in ‘North Korea’


 While comparing the pharmaceutical industry in Korea and Japan, the inter-Korean summit was held and I thought that not only Korea but also Japanese pharmacology college students would have questions about North Korea's pharmaceutical industry.


 ‘Pyongsu Pharma is the only pharmaceutical industry of North Korea. Pyongsu is a partnership between Pyongyang Pharmaceutical Company under the Ministry of Public Health of the DPRK(Democratic Peoples Republic of Korea) and Northern Development Pharmaceutical Consortium Ltd since 2002, producing and selling basic medicines such as analgesics and antibiotics in North Korea.


 Since it became operational in 2004, Pyongsu has been manufacturing both western and traditional Koryo medicines. The manufacturing activities are centered around tablets (up to 187 millions per year, per shift) & capsules (up to 57 millions per year, per shift). The main products are antibiotics such as amoxicillin, chloramphenicol, siprofloxacin, doxycycline, erythromycin, tetracycline, cotrimoxazol, and metronidazole. Based on the aspirin and paracetamol, they have also made Pyongsu Spirin and Pyongsu cetamol. In particular, while it has been very controversial about the hygienic environment of North Korea as the roundwarms were detected from the North Korean soldiers' body who defected to South korea through Panmunjom last year, Pyongsu pharma also produces anthelminthics with albendazole and mebendazole.


 It also has pharmacies and wholesalers throughout North Korea and provides investment services for warehousing, distribution, contracting, manufacturing and marketing.


 

 PyongSu currently counts 11 pharmacies in Pyongyang and Pyongsong as shown in the map above. Three pharmacies in Nampo, Wonsan and Hamhung have received the final permits and should open in 2018. Among the pharmacies located in Pyongyang, the pharmacy nearest to the statue of Kim Il Sung and Kim Jong Il is characterized by the fact that it runs 24 hours everyday.


 Pyongsu pharma received GMP certificate in 2007 and became a supplier for the International Federation of Red Cross (IFRC), allowing to participate in international tenders for the North Korean market.


UN Security Council (UNSC) Resolution 2375, adopted unanimously in September 2017, prohibits the opening, maintenance, and operation of joint ventures or cooperative entities with North Korean entities or individuals. A new resolution banning foreign joint ventures with North Korea have forced the company to close as it didnt secure a special allowance. The company had made requests of several U.N. member states seeking an exception for the sanction. Emphasizing human rights that there have been very serious shortages for a long time, particularly antibiotics, painkillers and other basic medicines. But, none of them has offered to do so. Pyongsus predicament shows the challenge world leaders face in trying to squeeze North Koreas ruling regime without further harming the isolated countrys endangered population.


4.Pharmaceutical company specialized in Kanpo (Tsumura)


 In Japan, there are some companies specialized in Kanpo. In Tsumura which account for 85% of the Kanpo marketing share, 95.4% of sales is prescribed Kanpo and 2.3% of sales is over-the-counter drugs. Kanpo is a medical technic which origin is came from China but developed originally in Japan.


Status of Kanpo used by doctors


 According to the survey conducted in 2011, doctors who use Kanpo now are 89.0%. It have increased by 5.5% compared with survey in 2008. More than 50% of doctors answered “Kanpo have high curative effects”. Also, as a reason doctors prescribe Kanpo, there are many voices such as “In the case that western medicine didn’t effect, Kanpo treatment was effective.”, “Patients asked me to prescribe Kanpo”.


How to secure the safe herbal medicines


 Herbal medicines, which are the base materials of Kanpo products are provided from China (about 80%), Japan (about 15%) and Laos (about 5%). In order to secure the safe and stable herbal medicines, Tsumura is proceeding to spread the cultivating land of the herbal medicines both inside and outside of Japan, and to strengthen the control of quality and storage capacity.


Business of prescribed Kanpo products


 In case of Kanpo products, its composition and manufacturing process has already known since more than 1,800 years ago. As there is no patent, everyone can produce freely. However, there are some problem in order to entry the prescribed Kanpo products business. One of the problem is that they need to prove biology equivalence with existing products. Kanpo drugs are multicomponent medicine, it’s difficult to provide generic drugs like single component medicines. Actually, there are no medications adopted as a generic medicine. Besides, someone need to store crude materials under the fixed environment.
