우연히 탄생한 탈모약

대표적인 탈모치료 전문의약품인 ‘피나스테리드’와 일반의약품인 ‘미녹시딜’에는 어떤 공통점이 있을까? 두 약 모두 탈모치료약을 개발하다가 발견된 것이 아니라, 기존 약의 부작용으로부터 우연히 발견되었다는 점이다.

‘피나스테리드’는 원래 전립선비대증 치료를 위해 개발되었는데, 탈모개선효과가 부작용으로 나타나면서 이 약은 주목받게 되었다. 탈모는 5알파환원효소(5α-reductase)에 의해 테스토스테론이 디하이드로테스토스테론(DHT)으로 전환되면서 발생한다. DHT가 두피 모낭을 위축시키고 가늘게 만들어 탈모를 유발하기 때문이다. 피나스테리드는 이 DHT를 만드는 5알파환원효소를 억제하여서 전립선비대증 뿐만 아니라 탈모에도 효과를 보인다. 그래서 피나스테리드 성분의 치료제인 ‘프로페시아’는 전립선비대증 치료제인 ‘프로스카’로 허가를 받은 5년뒤, 피나스테리드 용량을 조정하여 탈모치료제인 ‘프로페시아’로 재탄생하였다.

한편, ‘미녹시딜’은 1979년 원래 경구용 고혈압치료제로 개발되었다. 피나스테리드와 마찬가지로  발모현상이 부작용으로 나타나면서 탈모치료제로 재탄생하였다. 하지만, 미녹시딜은 피나스테리드와 다르게 머리뿐만 아니라 다른 부분에도 발모현상을 보여서 여성의 경우 수염이 나타나기도 한다. 이러한 이유로, 미녹시딜의 여성용 농도는 남성용인 5%보다 낮은 3%로 성별에 따라 농도가 다르며, 얼굴 이외의 부위에는 뿌리지 않도록 조심해야 한다.

 

어떤 탈모약이 좋을까?

캐나다 토론토 의과대학 피부과 전문의 아디티야 굽타 교수 연구팀이 피나스테리드, 두타스테리드, 미녹시딜의 발모효과를 비교한 결과, 두타스테리드 0.5mg을 하루 한 개씩 복용할 때 발모효과가 가장 크다고 보았다. 두타스테리드는 6개월 후 두발이 없던 부위에 솜털이지만 가장 많은 머리를 자라게 하였다.  2위로는 피나스테리드 5mg 하루 한 개, 3위는 미녹시딜 5mg 하루 한 개 순이다.

 

탈모약, 걱정되는 부작용 오해와 진실

피나스테리드가 탈모치료제로 사용될 경우 비급여가 적용되어 가격에 부담을 느끼는 사람들이 많다. 이 때문에 해외직구도 늘어나는 추세이다. 의사가 처방한 탈모약은 식품의약품안전처의 철저한 관리•감독하에 안전성이 보장되는 반면, 해외직구로 약을 구입하는 경우에는 어떠한 안전성도 보장할 수 없다. 해외직구로 구매하는 약은 제조공장의 위생상태를 알 수 없고, 소량의 중금속이 포함되는지도 알 수 없다. 이제는 특허가 만료되어 효능•효과는 그대로 유지하는 동시에 저렴한 제네릭 의약품도 많으므로, 꼭 안전성이 검증된 탈모약을 처방 받아 복용하길 바란다.
