COVID-19 백신, 어디까지 진행되었을까?

 

 

현재 모든 국가들이 COVID-19로 인해 골머리를 앓고 있다. WHO에 보고된 바에 따르면 전세계적으로 확진자가 1300만명을 넘어섰으며 사망자는 57만명 (WHO Coronavirus Disease Dashboard 기준)이다. 우리나라의 경우에도 이미 누적확진환자가 13500명을 넘어섰고(2020715일 기준) 289명이 사망했으며 아직까지도 30~60명 사이로 매일 확진자가 발생하고 있다. 이에 전 세계가 COVID-19 백신 개발에 마지막 희망을 걸고 있다. 세계 각국에서는 160개의 후보 물질을 놓고 백신개발에 열을 올리고 있으며 이 중에서 임상시험에 들어간 것만 벌써 23개이다. 통상적으로 백신 개발에는 최소 수년이 걸리지만 전례에는 없던 급박한 상황으로 인해 파격적인 지원과 규제 및 절차의 축소로 연내 백신이 공급될 것이라는 전망도 나오고 있다.

 

 

백신을 성공적으로 접종하게 되면 항체가 생겨나 다음에 그 병원체가 침입하였을 때 즉각 대응하는 체액성 면역 뿐 아니라 해당 병원체를 항원으로 기억하여 직접적으로 병원체를 공격하는 세포성 면역 두가지를 모두 유발할 수 있다. 백신은 이 두가지를 유도하는 것이 목표이며 이를 통해 질병 감염에 대한 예방 뿐만 아니라 집단 면역을 통해 집단 내 면역력이 약한 개인의 감염 확률을 낮출 수 있다는 의의를 가진다.
그렇다면 현재 개발 중에 있는 COVID-19백신은 어떤 원리일까? 현재 임상시험에 들어간 COVID-19 백신 후보는 크게 3가지 기전으로 분류되고 있다. 첫번째는 재조합 바이러스 벡터가 내인성 항원으로 작용하여 프로테아좀에서 가공된 후 MHC1에 표지 되어 독성 T세포의 세포 매개성 면역반응을 일으키는 기전이다. 두번째는 DNA 백신으로, 숙주세포 내에서 전사 및 번역된다. 이후 MHC1 경로를 따르거나 숙주 세포 외부로 이동하여 외인성 항원으로 작용함으로써 MHC 2에 표지 되어 T helper cell의 체액성 면역을 일으킨다. 마지막으로 세번째는 mRNA 백신으로, 숙주세포의 내포작용을 통해 들어가 앞서 언급한 DNA 백신과 같은 MHC2 경로를 통해 체액성 면역반응을 일으킨다.

 

 

 

이 중 가능성이 있는 가장 유력한 백신은 미국 바이오기업 모더나 사에서 개발중인 mRNA 백신인 ‘mRNA-1273’이다. COVID-19 바이러스는 스파이크 단백질을 미리 형성하여 사람의 수용체 단백질(ACE2)와 결합하여 침투하는 기전을 가진다. 모더나 백신 ‘mRNA-1273’은 이 스파이크 S 단백질을 암호화함으로써 체내에서 이에 대항하는 항체를 생성하도록 하여 침투 기전을 차단하는 원리이다. 모더나 사는 지난 3월 세계 최초로 COVID-19 백신 임상시험에 돌입하였으며 5월에 8명 참가자들의 체내 모두에서 바이러스를 무력화할 수 있는 중화항체를 형성하였다고 보고하였다. , 714일 뉴잉글랜드 의학저널(NEJM)에 임상1상 시험 결과 45명 전원에서 항체가 형성되었다고 발표하였다. 현재는 600명을 대상으로 한 임상 2상 시험을 실시 중이며 727일에는 3만명을 대상으로 의약품 출시 전 마지막 단계인 임상 3상에 돌입할 예정이다.

 

 

국내에서 COVID-19 백신 관련하여 진행중인 임상시험은 2건이다. 국제 백신 연구소에서 의뢰한 INO-4800과 제넥신에서 나온 GX-19을 모두 건강한 성인을 대상으로 1/2a상 초기 단계를 진행하고 있다. 식약처에 따르면 현재 임상시험 계획을 심사중인 COVID-19 백신 및 치료제는 5개이며 임상시험 신청을 위해 사전상담이 진행중인 백신은 10개 제품이 있다고 한다.

 

 

그러나 Financial times(FT)의 보도에 따르면 코로나 19에 대한 면역력 약화로 인해 백신이 갖는 효용성에 문제가 있다고 한다. 실제로 킹스 칼리지 런던 연구결과에 의하면 COVID-19에 걸렸다가 회복된 사람의 항체가 몇 달 내에 크게 감소하는 것으로 나타났다. 해당 연구 논문은 아직 동료학자들의 검토과정을 밟지 않을 상태이지만 백신이 얼마나 오랫동안 COVID-19를 예방할 수 있을 것인지에 대해 의문을 제기 한다. 코로나 바이러스 중 하나인 SARS 바이러스의 경우 항체 효과가 1~2년 간 유지되는 것을 보면 COVID-19 바이러스는 다른 코로나 바이러스들보다 항체가 훨씬 더 빨리 사라진다는 것을 의미한다. 이렇게 항체의 유지 기간이 떨어질 경우 현재 개발중인 백신 모두 처음부터 다시 시작해야할 것이라고 전문가들은 지적하고 있다.

 

 

또한, 백신이 만들어져도 완벽한 종식이 어렵다는 의견이 전문가들 사이에선 다분하다. 특히 모더나 사에서 개발중인 mRNA-1273이 성공하더라도 이후에 무용지물이 될 가능성이 있다고 한다. 창궐 초기에는 COVID-19 바이러스의 S 타입 스파이크 단백질이 주를 이루어 mRNA-1273S 스파이크 단백질을 이용하고 있지만 유럽 확산세를 겪은 이후 바이러스의 변이형태인 G 타입 확진자가 많이 발생하고 있다. 국내 확진자의 경우 대부분이 S 타입인 것으로 확인되고 있지만 유럽을 중심으로 퍼진 G 타입이 확산되는 추세를 감안하면 가을에 2차 재유행 시 G 타입 기반으로 확진자가 나올 가능성이 크다고 전문가들은 분석한다. 아직 S 타입 치료제의 G 타입 적용 가능 여부가 확인된 바 없어 COVID-19로부터 빠른 시일 내에 완전히 벗어나는 것은 힘들 수도 있다는 것을 시사한다. 그러므로 완전한 종식을 위해서는 한 나라에만 국한된 게 아닌, 글로벌 공동 노력이 필수적이며 새로운 변종에 대한 백신 개발을 위한 글로벌 기술 플랫폼을 확보할 수 있는 실질적인 지원에 대한 검토가 필요하다.

 

 

COVID-19 Vaccine, How far did it go?

 

 

Currently, all countries are troubled by COVID-19. According to a WHO report, the number of confirmed cases has already exceeded 13 million worldwide, with 570,000 deaths (based on the WHO Coronavirus Case Dashboard). Likewise, the cumulative number of confirmed cases has already exceeded 13,500 with 289 deaths (July 15th, 2020) in Korea and still 30 to 60 confirmed cases comes out every day. As a result, the world is pinning its last hopes on the development of the COVID-19 vaccine. Countries around the world are eager to develop vaccines on 160 candidate substances, 23 of which are already in clinical trials. Usually vaccine development takes at least several years, but due to the unprecedented urgency, some predict that vaccines will be supplied within the year because of the unconventional support as well as reduced regulations and procedures.

 

 

Successful Vaccination develops antibodies in the body that respond immediately when the pathogen invades the next time, which is called humoral immunity, as well as the cell-mediated immunity that directly attacks the pathogen by remembering it as an antigen. Vaccine aims to achieve both humoral immunity and cell-mediated immunity, which not only prevent infectious disease but also reduce the probability of infection in individuals with weak immune system via herd immunity.

The candidates for the COVID-19 vaccines currently under developed are classified into three main mechanisms. The first mechanism is a recombinant virus vectors which act as endogenous antigens, processed in protease, then labeled in MHCⅠ, causing cell-mediated immune responses concerning cytotoxic T cells. The second one is a DNA vaccine that are transcribed and translated in the host cell. The synthesized protein moves to the MHC class I pathway or would be released outside the host cell and act as a exogenous antigen and are presented by MHCⅡ to the T helper cell leading to humoral immune response. The last one is a mRNA vaccines that are endocytosed leading to MHC class Ⅱ pathway which is same mechanism with DNA vaccine and causes humoral immune response.

 

 

The leading candidate of vaccine is the mRNA-1273, an mRNA vaccine being developed by U.S. bio firm Moderna. The COVID-19 virus has a mechanism of pre-forming spike proteins which combines to the human receptor proteins (ACE2) and infiltrate a host cell. Moderna vaccine 'mRNA-1273'  blocks this penetration mechanisms by encoding spike S protein to produce antibodies against the virus in the host body. Moderna launched the world's first COVID-19 vaccine clinical trial in March and reported in May that it had formed a neutral antibody in all eight participants. In addition, on July 14, the New England Journal of Medicine (NEJM) reported that antibodies were formed in all 45 participants in the phase Ⅰ trial. It is now conducting phase Ⅱ trial on 600 people and will begin phase Ⅲ on 30,000 people , which is the final phase before the drug's release, on July 27.  

 

 

In Korea, there are 2 ongoing clinical trials related to COVID-19 vaccines. INO-4800 commissioned by the International Vaccine Institute and GX-19s from Genexine are in the initial stage of phase 1/2a trial on healthy adults. According to the Ministry of Food and Drug Safety, there are 5 COVID-19 vaccines and treatments that are under review for clinical trial plans, while 10 vaccines are under pre-consultation to apply for clinical trials.

 

 

However, according to a report from Financial Times (FT), there is a problem with the effectiveness of vaccines due to weakened immunity to COVID-19. In fact, the London study of King's College found that the amount of antibodies from people who had suffered from COVID-19 and recovered greatly decreased within a few months. Although it has not yet taken the peer review process the research asks questions about how long will the vaccine be able to prevent COVID-19. In the case of SARS virus, which is one of the corona viruses, the antibody effect has maintained for a year or two, which means that the antibodies against COVID-19 virus disappears so much faster than the other coronaviruses. Experts point out that If the maintenance period of antibody is not long enough, all the vaccines that are now in development will have to start all over again.

 

 

Many experts also say that a complete end is difficult even if vaccine is successfully created. In particular, even if the mRNA-1273 from Moderna Corporation succeeds, it may become useless later on. In the early days of the outbreak, the S-type spike protein of the COVID-19 virus was the main source, therefore mRNA-1273 also used S- type as well. However since the outbreak of the European epidemic, many confirmed cases of G-type have been occurred. Although most of the confirmed cases in Korea are identified to be S-type, experts say that considering the spread of G-type, which is now prevalent in Europe, there is a high possibility that the confirmed cases will be based on G-type in the fall season. It has not yet been verified whether S-type treatments are applicable to on G-type, suggesting that it may be difficult to completely deviate from COVID-19 in the near future. Therefore, a complete end requires global efforts not limited to one country, and a review of practical support is in need to secure a global technology platform for vaccine development which enables us to confront the new strains.
