과거에는 약사라고 하면 약국에서 근무하는 약사들을 떠올리곤 했지만, 최근에는 약학대학을 졸업한 후 약국, 병원, 공직, 회사 등 다양한 진로에 대한 관심이 높아지고 습니다. 이번 기사에서는, 한국과 일본에서 약학대학 졸업생들이 어떤 진로로 진출할 수 있는지, 각 직능에서 어떤 역할을 수행하고 있는지 간략하게 짚어보았습니.

 

공직 약

 

 공직약사국가나 지방자치단체에서 의약품과 관련된 업무에 직접적 또는 간접적으로 종사하는 공무원으로서, 약사 면허를 가지고 있는 사람으로 정의할 수 있습니다. , 약사 인력관리, 약국/도매상/의약품 제조업소에 대한 관리 및 감독, 의약품 인허가, 건강보험 약가제도 등 의약품과 관련된 공적인 업무를 보는 약사입니다.

 2013 대한민국 행정자치부에서 발표한 공무원총조사에 따르면, 공무원 중 약사면허 소지자는 총 779으로, 이들이 공직약사에 해당합니다. 한국에서는 매우 다양한 중앙부처와 공공기관에서 약사가 근무하고 있는, 특히 보건복지부, 식품의약품안전처, 특허청 등의 중앙부처와, 건강보험심사평가원, 국민건강보험공단, 한국의약품안전관리원 등 공공기관에 약사의 비율이 높은 편입니다.

 이러한 공직에서 약사가 하는 역할 크게 3가지로 나눌 수 있습니다.

  첫째 약사법, 마약류, 화장품, 국민건강보험법과 같은 법령을 제정 또는 개정하는 일, 둘째 의약품 관련 제도가 잘 적용되고 있는지, 개선할 사항은 없는지 검토하고 방향성을 잡는 일,

셋째 법령이 잘 시행될 수 있도록 관리, 감독하고 예산을 집행하는 일입니다.

 일본에서의 공직약사는 한국에서의 역할보다 많이 축소되어 있다고 합니다. 공직에 종사할 경우와 지역약국에 근무할 경우 월급이 차이가 많은 것이 큰 이유입니다. 한국도 같은 이유로 공직에 근무하는 약사가 적은 편이지만, 최근에는 삶의 질이 중요시되면서 근무 환경이 좋고, 연금이 지급되는 공직에 대한 관심이 증가하는 추세입니다.

 

 

제약회사 약사

 

 한국과 일본에서 제약회사 약사가 수행하는 업무는 매우 유사한데요, 약사로서 회사에서 근무할 수 있는 부서는 매우 다양하고 회사마다 부서의 이름도 다르지만, 제약회사에서 약사의 주된 역할을 신약 개발의 과정에 따라 구분할 수 있습니다.

 

 첫째, 약사는 연구소에서 신약 후보물질을 탐색거나, 의약품의 개량을 위해 개발연구를 직접 수행합니다. 대부분의 경우, 연구소에는 학부를 졸업하고 석사 이상의 학위를 취득하여야 연구원으로 일할 수 있으므로 약학 외에 생물학, 화학 전공자도 많은 편입니다.

 

 

 

 

둘째, 유효 물질을 발견하고 독성이 검증되고 나면 임상시험을 진행합니다. 임상시험 단계에서 약사는 의약품의 효과를 적절히 검증할 수 있도록 임상시험을 설계하는 업무부터, 임상시험 진행 준비, 임상시험이 관리규정에 따라 실시되고 있는지 확인하는 업무를 합니다. 약사는 시험하고자 하는 의약품의 특성에 맞게 임상시험을 적절히 설계할 수 있고, 임상시험 관리에서도 약학적 지식을 다방면에서 활용할 수 있기에 적합한 직능이라고 할 수 있습니다.

 

셋째, 임상시험을 통과하고 의약품 판매를 허가 받는 단계에서, 약사는 약의 허가 받기 위한 등록 전략을 수립하고, 등록 자료를 준비하는 RA역할을 합니다. 또한 한국에서는 급여항목으로 등록된 의약품만이 국민건강보험이 적용될 수 있기 때문에, 급여 등록 신청을 하고, 적정한 약가를 협의하는 업무를 할 수 있습니다.

 

넷째, ‘제약회사의 꽃이라고도 불리는 영업, 마케팅 업무가 있습니다. 회사에서 의약품의 판매허가를 받으면, 마케팅 부서에서는 소비자를 설득할 전략을 수립하고, 영업 부서에서는 이를 바탕으로 직접 소비자들을 만나 홍보활동을 하고, 피드백을 수렴하는 역할을 합니다. 이 업무에서도 의약품에 대해 잘 아는 약사가 과학적 근거를 바탕으로 잘 설명할 수 있습니다.  주로 약학대학 학생이 마케팅분야로 원하는 경우, 영업에서 시작하여 경력을 쌓고, 경험한 후 마케팅으로 옮기는 것이 흔합니다.

 

다섯째, 의약품의 개발부터 시판 후까지, 안전성 정보를 수집하여 임상시험에서 발견하지 못한 부작용은 없는지 확인하는 Pharmacovigilance 있습니다. 한국에서는 제약회사에 안전관리책임자를 필수로 두게 하고, 2014년부터는 주기적으로 교육을 실시하여 제품의 safety profile 관리를 중요하게 여기고 있습니다.

 

일본은 제약산업에서 한국보다 앞선 선진국으로 알려져 있습니다. 일본 제약회사에서 한국과 다른 점 공장에서 약을 생산할 때 대부분 일본에서 생산되는 국산 기계를 사용한다는 점입니다. 특히 약의 품질을 결정하는 타정기의 경우, 한국 대부분 수입에 의존하는 반면 일본의 경우에는 자국의 타정기 생산 기술을 가지고 있습니다. 그리고 의약품 원료의 경우에도 수입에 의존하기 보다는 대부분 자국 원료를 사용하고 있습니다. 그리고 수요가 적은 약은 직접 생산하기보다는 약을 외국에서 수입하여 환자들에게 공급하는 체계를 가지고 있습니다.

 

 

병원약사

 

병원약사는 의료기관 약제부에서 근무하는 약사들을 일컫는 용어입니다. 한국의 병원약사는 크게 전문약사가 되기 전 담당 전공 분야에서 수련하는 '전공약사', 병원 안에서 조제업무를 담당하는 '정규약사', 임상을 전문적으로 담당하는전문약사로 나누어집니다.

2017년 대한약사회의 통계에 따르면 약사들 중 병원약사는 15%이지만, 중환자와 밀접하게 관계를 맺고 전문성을 키울 수 있어 큰 만족감을 느낄 수 있는 직종입니다. 한국의 6년제 약대 졸업생들에게 희망진로 선호도 1위를 수년간 지킬 정도로 인기가 많습니다.

 

한국에서는 병원약사를 3가지로 나눌 수 있습니다. 정규약사는 병원에서 대부분의 조제업무를 담당합니다. 환자들의 처방을 검토하고 처방에 따라 약을 조제한 후 복약사항을 지도하는 역할을 합니다. 약국약사와 비슷한 역할을 맡지만 병원 내의 응급환자를 비롯한 외래환자와 입원환자를 대상으로 한다는 차이점이 있습니다

 

전공약사는 실무수련을 거쳐 임상에 전문화된 약사로 양성되는 단계의 수련 약사입니다. 하나의 전공을 선택하거나 병원 전체를 순회하며, 평균적으로 1년간의 수련과정을 거치게 됩니다. 병원에 따라 수련기간 동안 약간의 조제 업무도 담당하기도 하지만 일과의 대부분을 임상 수련에 집중합니다. 하지만 공부량과 업무량이 매우 많기 때문에 수련을 견디지 못하고 이탈하는 인원들도 상당하다고 합니다.

 

 전문약사는 자신의 전문 분야를 통달하고 약물요법에 관해 보다 전문적인 자질과 능력을 갖춘 임상약사입니다. 전문약사가 되기 위해서는 응시자격을 갖춘 후 전문약사시험에 응시하여 합격하면 자격이 발부됩니다. 최근 전문약사들이 임상전담약사로서 의료진들과 협진하는 '다학제 팀의료'가 한국 주요 병원에서 정착되고 있습니다.

 

 이해를 위하여 병원약사를 3분류로 나누었지만, 사실 병원에 따라 그 구분 경계가 명확하지는 않습니다. 전공약사만 모집하는 약제부도 있고, 원내 수련 과정이 없어 정규약사만 모집하는 약제부도 있는 등 병원의 필요에 따라 구성 비율의 큰 차이를 보이고 있습니다. 실제로 한국에서 전공 약사 과정이 개설된 병원은 6곳 뿐입니다.

 

 일본에서 병원약사 약의 조제, 병원내 제제 준비, 주사제 준비, 의약 정보 제공, 병동에서의 의료서비스 제공, 외래환자 복약상담 그리고 외래 화학치료실 약사 등7가지 역할을 수행합니다.

 일본의 병원에도 한국과 같이 조제실이 있으며,

 

1)입원환자와 외래환자에게 처방전에 따라서 약을 조제합니다.

 

2)‘병원내 제제란 약사가 입원 환자 개개인에게 효과적이고 안전할 수 있도록 용량, 용법을 조절하여 준비하는 약으로, 약에 대한 전문가인 약사의 지식이 꼭 필요한 일입니다.

 

3)직접 혈관으로 주사되어 뛰어난 효과를 내는 만큼 정확한 용량 측정이 필요한 주사제의 경우, 약사는 환자의 중증도에 따라서 용량을 고민하고, 준비하는 역할을 합니다.

 

4)또한, 병원에서 의사나 간호사들에게 약의 부작용이나 상호작용, 적절한 사용법 등을 조언하고 과학적 근거를 제공하는 일 또한 약사의 업무입니다.

 

5)일본에서는 병동에서 약사가 제공하는 의료서비스가 많이 있습니다. 약사는 입원 환자나 가족과의 면담을 통해 복용중인 약물, 건강식품 등을 확인하고 상호작용과 부작용을 체크합니다. 또한 약의 투여용량이나 투여율에 이상이 없는지, 주사제와 경구 약물의 조합이 환자에게 적절한지, 효과가 잘 나타나는지 꾸준히 모니터링 함으로써 환자가 안전하게 치료받을 수 있도록 돕습니다.

 

6)병원에 외래 환자가 방문 할 경우, 약사는 환자가 다른 의료기관에서 처방 받은 약은 없는지, 약을 복용하는 동안 문제는 없었는지 확인합니다. 약사는 이러한 복약 상담 결과를 의사와 간호사들에게 전달하여, 다음 방문 시에도 안전하고 효과적인 치료가 가능하도록 합니다.

 

7)마지막으로, 약사는 병원의 외래 화학 요법실에서 항암 치료 계획서를 확인하고, 항암제를 준비하고, 환자에게 항암제와 부작용에 대해 충분히 설명하여 환자가 안전하게 치료받을 수 있도록 합니다.

지역약국 약사

 

한국과 일본의 지역약국 약사의 역할에 앞서, 약국 형태의 차이를 먼저 알아보고자 합니다. 일본은 큰 대학병원과 작은 병원(클리닉)의 진료비가 비슷합니다. 그래서 큰 대학병원 앞에는 체인 방식의 대형 Community 약국많고, 러한 약국은 약사들을 고용하여 운영 됩니다. 예를 들어, 후쿠오카의 한 community 약국의 경우 20명의 약사가 한 약국에 근무하고 있으며, 근무년차에 따라서, 임금이 증가합니다. 또한 일본에는 한국 관광객들에게 널리 알려진 돈키호테처럼, 약국내에 의약, 일반의약품과 다양한 물품들을 취급하는 드러그스토어형태의 약국이 많습니다. 마지막으로, 일본에는 한약국이 있습니다. 한국은 약사가 아닌 한약사라는 직능이 따로 있어서 한약국을 운영하지만, 일본에서는 한약사가 따로 없고, 약사가 한약을 취급하는 자격증을 딴다면 한약을 취급할 수 있습니다.

한국약국 약사가 개인사업자로 등록해 개국하는 '개인약국', 프랜차이즈 업체와 제휴하여 운영하는 '프랜차이즈 약국', 대기업에서 약사를 고용 혹은 개설하도록 하는 '드러그스토어 약국'이 존재합니다. 한국에서는 '1 약사 1 약국'을 원칙으로 하기 때문에 의약품을 취급하는 드러그스토어의 비중은 매우 낮고 개인약국과 프랜차이즈 약국이 높은 비중을 차지하고 있습니다. 한국에서 흔히 볼 수 있는 드러그스토어인 '올리브영'은 엄밀히 말하자면 의약품을 취급하지 않고 화장품과 건강기능식품을 주로 취급하는 H&B(Health & Beauty) 스토어입니다. 또한 한국 약국명 중 흔히 볼 수 있는 '온누리'라는 상호는 한국에서 가장 큰 프랜차이즈 상호입니다. 프랜차이즈 약국은 친숙한 브랜드 효과뿐만 아니라, 약국 운영 노하우 전수, PB상품 구비 등의 강점을 가지고 있습니다.

 

 

약국약사의 역할

 

한국에서는 의사 진료와 처방을 맡고 약사는 조제와 지도를 담당하는 의약분업이 시행되고 있습니다. 환자는 병원에서 진료를 받은 후, 처방전을 발급받아 약국에 들러 약을 받습니다. 따라서 약국약사의 기본적인 업무들은 의사의 처방전을 기반으로 하여, 처방전을 감사하고 약을 조제한 후에 환자에게 복약지도를 하는 일련의 과정이 이루어집니다.

  약국에서는 처방이 필요한 전문의약품(ETC, Ethical The Count)에 대해서는 처방전 건 당 조제료, 복약지도료 등의 의료수가에 따라 국민건강보험공단으로부터 수익을 얻습니다. 이외에도 처방전이 필요없는 일반의약품이나 의약외품들(OTC, Over The Count)을 상담하고 판매하는 매약도 중요한 업무입니다. 약국약사는 국민과 가장 가까운 고에서 의약품의 적절한 사용과 자가치료에 필요한 정보를 제공하고, 공중보건증진에도 기여합니다.

 

 일본에서는 환자로부터 처방전을 받아 약을 조제하고, 복약지도 하는 부분에 있어서는 한국과 동일합니다. , 일본에서는 의사의 결정에 따라 환자가 처방약을 병원에서 받기도 하고, 약국에서 받기도 합니다. 일본과 한국의 큰 차이점은 방문약사입니다.

JAPAN

본은 한국보다 더 먼저 출산율이 감소하고, 60세 이상의 인구가 증가하여 고령화사회에 진입하였습니다. 그래서 대부분의 지역약국 약사들은 직접 환자들의 집을 방문하는 방문약사시스템이 활성화되어 있습니다. 후쿠오카의 큰 지역약국의 경우 연 평균 159회정도 약사들이 직접 집을 방문하여 거동이 불편한 노인들이나 환자들을 관리하고 있는데, 환자들이 약을 적절하게 복용하고 있는지, 상태는 어떠한지 확인합니다. 이러한 방문약료는 약에 대한 이해도를 높이는 것은 물론 약화사고를 줄일 수 있으며, 방문과정에서 얻은 정보를 의사 또는 환자를 돌보아주는 분들과 공유하여 환자가 더 밀접하게 관리 받을 수 있게 해 줍니다.

두 번째 차이점은 복약상담비 입니다. 한국의 경우에는 복약상담에 대한 의료수가가 천원 남짓이지만, 일본은 10배정도에 달하는 복약상담비를 받습니다. 그만큼 복약지도의 비중을 중요하게 보는데, 일부 약국에서는 모니터를 활용하여 환자들이 먹은 약에 대해 일일이 상담해주기도 하고, 건강 상담을 해주기도 합니다

세 번째로, 일본 약국에서는 정부로부터 허가 받은 몇 가지 성분에 한해서는 환자들이 원하는 경우에 처방전이 없이도 약을 제조할 수 있습니다. 약값이 비싸다는 단점이 있지만, 과가 뛰어나 환자들이 많이 찾고 있습니다.

 마지막으로 일본에서는 처방전에서 칸포가 차지하는 비율이 큽니다. 일본식 한약을 칸포라고 하는데, 양약과 칸포를 함께 처방함으로써 양약이 커버하지 못하는 부분을 칸포가 보완해줄 수 있다고 합니다.

 

일본에서 특별한 약사의 역할 - 학교약사

 

 한국과 달리, 일본에는 학교보건안전법에 따라 대학을 제외한 유치원, 초등 학교, 중학교, 고등 학교에 근무하는 약사가 있습니다. 학교 약사는 학생들의 위생 환경을 점검하고, 학생들이 혹시 약물이나 알코올, 흡연을 하지는 않는지 확인하는 역할을 합니다. 그 외에도 다음과 같은 역할이 있는데, 성장기의 학생들에게 가장 필요한 건강 파트너라고 할 수 있을 것 같습니다.

However, after graduating from a college, there are various careers such as pharmacy, hospital, public office, and company. In this article, we will briefly discuss career paths for graduates of pharmacy colleges in Korea and Japan, and what they are doing in each function.

 

 

 

 

 

Public Pharmacist

 

 

 Maybe you heard about 'public official', but what about 'public pharmacist'? According to the 2016 Pharmacists Statistics published by Korean Pharmacists Association (KPA), the pharmacists who work in the public service accounted for 0.19% among the member of the KPA. As the report shows the public pharmacist would be the most unfamiliar pharmacist in Korea. However, the recent interest is increasing because its working condition and worthwhile. What does a public pharmacist in Korea do?

 

 First, 'public pharmacist' can be defined as a person who has a license for a pharmacist who is directly or indirectly engaged in the work related to medicines in a country or local government. That is, it is a pharmacist who observes the official work related to pharmaceutical products such as pharmacist supply management, management and supervision of pharmacy / wholesaler / pharmaceutical factory, drug license and health insurance drug price system. According to the results of a survey released by the Ministry of Government Administration and Home Affairs in 2013, 779 public officials have pharmacists license. There are various central departments and public institutions where pharmacists usually work. In particular, the central ministries such as the Ministry of Health and Welfare, the Food and Drug Administration, and the Patent Office have many pharmacists. If you are a student who thinks of a public pharmacist, youd be better to think about what you want to do for the national health and medical care and welfare, and then you might want to choose the right institution for your job.

 

 In one article, author said that public pharmacists are to judge and choose what is desirable in public pharmacy business. Although the 'pharmacist' is being mentioned as a job that will disappear in the future due to the 4th industrial revolution, the pharmaceutical bio industry will grow and the interests will become more complicated. Therefore, public pharmacists who judge the value of a desirable drug will still be an important occupation.

 

 

Pharmaceutical company pharmacist

 

 Recently, many Korean pharmacy students are interested in pharmaceutical companies. There are many departments that can work in the company as a pharmacist. But I would like to find out some of the main roles of pharmacists in pharmaceutical companies according to the process of drug development, as each company has a different departments name.

 

 First, the pharmacist conducts research at the laboratory to search for new drug compound candidates, or to improve pharmaceuticals. Since a person with a master's degree can work as a researcher in most case, there are many biology and chemistry majors apart from pharmacists.

 

 Secondly, after finding effective substances and testing for toxicity, clinical trials are carried out. During clinical trials, pharmacists are responsible for designing clinical trials to ensure the effectiveness of the drug, preparing for clinical trials, and conducting clinical trials in accordance with regulatory requirements. Pharmacists can design clinical trials appropriately according to the characteristics of the drug to be tested, and are able to utilize their pharmaceutical knowledge in many aspects of clinical trial management.

 

 Third, at the stage of passing clinical trials and licensing medicines, pharmacists establish registration strategies to obtain licenses and act as RA to prepare registration materials. Also, in Korea, only drugs which registered as payable items can be covered by National Health Insurance, so pharmacists can apply for payroll and negotiate appropriate drug prices.

 

 Fourth, there is sales and marketing business called 'flower of pharmaceutical company'. When a company receives permission to sell a drug, the marketing department establishes a strategy to persuade the consumer, and the sales department engages in public relations activities and receives feedback based on this. Pharmacists who are familiar with pharmaceuticals can also explain this well based on scientific evidence. If you want to be a marketer, you have to start with sales, build up your career, and then move on to marketing.

 

 Fifth, there is a Pharmacovigilance(PV) role that collects safety information from the development of pharmaceuticals to post marketing step. PV pharmacists make sure that you do not have any side effects that were not found in clinical trials. In Korea, since the year 2014, the safety management officer has become a mandatory requirement for pharmaceutical companies. They are constantly monitoring the safety profile of products. The PV business is becoming popular especially with the recent hiring of pharmacists.

 

 Pharmacists in pharmaceutical companies can do a variety of tasks, but their work is not confined to one department, and there is also active collaboration among departments. For example, the marketing department works with the MSL department to review the objectivity and validity of materials when producing materials for distribution to patients or physicians.  The 21st century is the age of pharmaceuticals and biotechnology, so the size of pharmaceutical companies is growing, and the interest about safety is growing. (1)   Dispensing

In-hospital formulation is a formulation prepared in a hospital by a pharmacist and used in that hospital. (3)   Injection preparation

Injectable medicines are generally highly effective and are administered directly into the blood vessels, so special care is required for their use. And information is provided to medical staff such as doctors, pharmacists, nurses, and patients.
